VHAPM Part 806.3 Other Than Full and Open Competition (OFOC) SOP Attachment 2: Request for Sole Source Justification Format >$150K OFOC SOP Revision 06 Page 6 of 7 Original Date: 03/22/11 Revision 06 Date: 08/30/2017 DEPARTMENT OF VETERANS AFFAIRS Justification and Approval (J&A) For Other Than Full and Open Competition (>$150K) Acquisition Plan Action ID: VA242-17-AP-6745 Contracting Activity: Department of Veterans Affairs, VISN 2, VA New Jersey Health Care System submits this justification for Other Than Full and Open Competition represented by purchase request 561-18-2-6046-0001 for Planetree Model Patient-Centered Care Consultant Services. Nature and/or Description of the Action Being Processed: This is a request for sole source authorization for VA New Jersey Health Care System (VANJHCS) to continue a membership affiliation allowing for consultative services with Planetree, Inc. for a base period January 1, 2018 through December 31, 2018, with the possibility for four (4) option periods. Planetree is a non-profit organization that provides education, resources, and information to healthcare organizations to facilitate efforts to create patient- and resident-centered care environments in order to attain Joint Commission Recognized Designation for Hospitals. This is a request to continue the existing affiliation with Planetree, Inc., currently under contract VA243-13-C-0010 that is set to expire December 31, 2017. Through Planetree, VANJHCS has achieved both Bronze and Silver Level Tier JC Recognition. VANJHCS has set a goal to reach the Gold Level Designation recognized by the Joint Commission. This authorization will allow VANJHCS to continue a membership affiliation to include consultative services at an annual estimated cost of $59,825.00; $37,000.00 representing membership dues and $22,825.00 for consultative services. VANJHCS expects to reach the Gold Designation within two to three years where additional years may be required in case the process takes slightly longer. Both membership and consultative services are a requirement of Planetree s designation process. Consultative services provide feedback on the status of the designation. VANJHCS is requesting approval to continue the Planetree membership and consultative services in an effort to reach this milestone. Description of Supplies/Services Required to Meet the Agency s Needs: Planetree Inc. was founded in 1978 and has a history of partnering with hospitals, healthcare organizations, and providers across the continuum of care to transform organizational cultures and improve the patient experience. Presently, Planetree s international membership is comprised of more than 500 originations from various countries. VANJHCS has been working with Planetree Inc. since 2005. Planetree was selected in 2010 by the Department of Veterans Affairs (VA) to collaborate with the VA Office of Patient-Centered Care and Cultural Transformation in the development of the VA s model for Veterans who receive health care service at VA s more than 1,000 point of care locations across the nation. Planetree provides all resources necessary for helping VANJHCS continue the cultural transformation and patient-centered care principles by providing: 1. Membership in an alliance of health care facilities and individuals that have incorporated Planetree Principles and Model of Care into their organizations, with the goal of achieving patient-centered care certification through a quality award that is recognized by Joint Commission. 2. Direct on-site consultation and guidance at all levels for patient satisfaction, design, progress assessments, determination of readiness for designation applications, etc. 3. Comprehensive network of resources in patient-centered principles i.e. newsletters, conferences, educational offerings and webinars, online support community, resource library, etc. This requirement is in line with the VHA Strategic Plan, Mission, Vision, Principles, Goals and Objectives to be Patient Centered, Team Based, Data Driven/Evidence Based, Continuously Improving, etc. https://www.va.gov/health/docs/VHA_STRATEGIC_PLAN_FY2013-2018.pdf Planetree provides the resources necessary to continue the cultural transformation and patient-centered care principles such as coaching and consultation services, direct on-site consultation and guidance, staff education and training, feedback on process, ongoing advice and consultation to leaders at all levels, patient satisfaction assessments, design consultations, etc. The estimated cost of this service is $62,000.00 from February 1, 2018 through January 31, 2019, plus four (4) option year periods for a total cost of $310,000.00. Planetree is the only entity that offers a Joint Commission recognized patient-centered care certification program across the continuum of healthcare to include acute care, behavioral health, long term and continuing care, and outpatient care. The Planetree Patient-Centered Certification builds on its already established tiered recognition including Bronze and Silver levels to indicate progress in patient-centered care. Bronze Recognition for Meaningful Progress in Person-Centered Care Silver Recognition for Significant Advancement in Person Centered Care Gold Certification in Patient/Person Centered Care The VA NJ Healthcare System is seeking Patient-Centered Care Certification at the Gold level offered by Planetree. VA NJHCS has already achieved Bronze and Silver recognition and the final step is to apply for full facility Planetree Patient-Centered Care certification in all continuums of care. The Planetree Patient-Centered certification program provides a practical, operational framework for evaluating the organizational systems and processes necessary to sustain a patient-centered culture. In order to achieve certification, the healthcare system must undergo a rigorous, yet collaborative, process to verify that it has successfully implemented programs that meet the spirit and intent of the designation criteria. Planetree s Patient-Centered Care Designation/Certification is the only program listed on the Joint Commission s Quality Check websites https://www.jointcommission.org/facts_about_quality_check_and_quality_reports/ and https://www.qualitycheck.org/glossary/#Behavioral_Health_Care: Patient-Centered Designation Program: The Planetree Patient-Centered Designation Program recognizes hospitals that have embraced and implemented patient-centered care in a comprehensive manner, improving both clinical and patient satisfaction outcomes. Based on the core elements of a personalized, humanized and demystified approach to healthcare, 54 specific criteria must be met in order to demonstrate that a site has implemented that dimension of the patient-centered culture. The program is designed to celebrate achievement, as well as to provide a concrete and measurable framework for comprehensive implementation of patient-centered care. In this spirit, the criteria a healthcare organization must meet focus on both process and outcomes. The criteria reflect the experiences of hospitals that have been engaged in an ongoing, focused effort to cultivate a culture of patient-centered care over a period of several years. The 54 Planetree Patient-Centered Designation Program criteria are categorized within the following areas: Structures and Functions Necessary for Implementation, Development, and Maintenance of Patient-Centered Concepts and Practices 2. Human Interactions 3. Patient Education and Access to Information 4. Family Involvement 5. Nutrition Program 6. Healing Environment: Architecture and Interior Design 7. Arts Program 8. Spirituality and Diversity 9. Integrative Therapies 10. Healthy Communities 11. Measurement Compliance with the designation criteria is verified through an assessment process that includes a written self-assessment, focus groups with patients, leadership and staff, and a review of outcomes measures. The Planetree Patient-Centered Designation Program was created by Planetree, a not-for-profit organization founded by a patient that has been working with healthcare providers for more than 30 years to personalize, humanize and demystify healthcare for patients. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: Solicitation VA242-17-Q-1153 was solicited unrestricted, and posted to GSA e-Buy on September 15, 2017 with responses due October 13, 2017. Source selection would be the lowest priced technically acceptable offer. The solicitation resulted in one offer; from The Grant Group, LLC., however their price was determined not fair and reasonable. NAICS code 541611 with keywords Planetree Model and Joint Commission searches were performed onVetBiz.gov and SBA.gov (DSBS), respectively, resulting in numerous sources. On November 14, 2017 a Sources Sought was posted to FBO.gov, with capability and business size responses due November 28,2017. Only one response was received from Zo Brown Speaks, LLC, a small minority business, however their capabilities statement did not reveal they were capable of performing this requirement. Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: The expected annual cost for this requirement is approximately $59,825.00; consisting of $37,000.00 for membership dues and $22,825.00 for consultative services. The cost is not expected to change from FY2017, nor is it expected to change over the following four (4) option periods. If procured unrestricted, the government may expect to incur duplication of costs and delays for another organization to assess the current process and needs going forward. In addition, switching to a model of care offered by another organization may require total rework and additional investment beyond the membership and services offered by continued membership and affiliation with Planetree, Inc. As a result, the Contracting Officer has deemed a total expected contract price of $299,125.00 to be fair and reasonable. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: Companies reviewed by the program office include National Committee for Quality Assurance (NCQA), Press Ganey, Patient-Centered Outcomes Research Institute, Beryl Institute, Leapfrog Group, the Pioneer Network, Cipher Health, The Institute for Healthcare Improvement, One View, and Planetree, Inc. Press Ganey currently offers census-based surveying and consultative services, but no type of facility certification in patient-centered care. Patient Centered Outcomes Research Institute funds research to help patients and those who care for them make informed healthcare decisions. The Beryl Institute currently offers educational services and an annual conference, but does not offer facility certification in patient-centered care. Leapfrog Group collects, analyzes, and disseminates hospital data to inform value-based purchasing. The Pioneer Network offers networking, educational services, and a conference, but not facility certification in patient-centered care. Cipher Health offers technology services to improve the patient experience. The Institute for Healthcare Improvement offers education resources and tools, but not a certification program. One View Healthcare offers technology services to improve the patient experience. Besides Planetree, only NCQA offers any form of Patient-Centered Care Recognition or Designation; however, this designation is only applicable to the Primary Care Medical Home setting and the certification is not recognized by Joint Commission with a Quality Check Award. Any Other Facts Supporting the Use of Other than Full and Open Competition: VA NJ Health Care System serves as the VHA Region 4 Patient- Centered Care Center of Innovation for VHA facilities located in VISNS 1, 2, 4, 10 and 11; and is leading the VHA cultural transformation for personalized, proactive and patient-driven health care. During 2017, VA New Jersey Healthcare System-Lyons Campus was awarded Planetree Silver Recognition for Significant Advancement in Patient-Centered Care for the continuing care environment. Receiving this prestigious honor, the continuing care environment at Lyons joins Acute Care, Outpatient Care, and Behavioral Health within our healthcare system for achieving Silver-level recognition. VA New Jersey Healthcare System is one of only five healthcare organizations worldwide who have achieved Silver-level recognition since Planetree first introduced the recognition level in 2012. The next step is to submit an application for Planetree Patient-Centered Certification throughout all environments of care in the NJ Healthcare System. VA New Jersey Health Care System also hosted staff from the VHA Office of Patient-Centered Care and Cultural Transformation, and was designated as a flagship site for Whole Health resulting in receipt of $85,290 from the Office of Patient Centered Care to implement Whole Health programming involving horticultural therapy and culinary education. Utilizing the Planetree model in VANJHCS is beneficial to Veterans as well as to the VHA s organizational mission and vision to continuously improve to strive for excellence in patient-centered care. The facility has created the FTEE to support patient-centered care implementation and adoption of best practices. The VA NJ Healthcare Center is committed to patient-centered care and the Planetree model has enabled achievement of a national level of recognition within the VA as a Center of Innovation for Patient-Centered Care. Additionally, VA NJ Health Care System has received accolades from the private sector for patient-centered care initiatives, including Honorable Mention from the 2011 Governor s Environmental Excellence Awards in the Healthy and Sustainable Communities Category, Practice Greenhealth Partner Recognition Awards, and Planetree best practices awards for healthy communities, design, healing arts, and enhancing the experience of sleep. Continued membership affiliation with Planetree shall maintain the existing level of support services that has facilitated VANJHCS s performance at this exceptional level. Maintaining the membership offers cost effective benefit to the government that include free access to webinars and educational materials to facilitate Patient-Centered Care Certification, a waived certification application fee, reduced fees for on-site evaluations, and a discounted conference fee. Additionally, member benefits include an individualized plan for cultural transformation that adapts to the needs, history, and priorities of the organization through a tailored selection of Planetree s services, access to a dedicated Planetree adviser who guides the facility s journey, and access to the entire Planetree network to gain knowledge and transfer ideas. Most importantly, the VA New Jersey Healthcare System has the opportunity to be recognized within an elite group of organizations world-wide by achieving Planetree s unique Patient-Centered Care Certification that no other agency can provide as recognized by Joint Commission. Listing of Sources that Expressed, in Writing, an Interest in the Acquisition: The Mihalik Group, LLC and SIGMA Health Consulting showed interest in response to RFI to GSA VA242-17-N-0956, however, they did not submit an offer in response to FSS RFQ VA242-17-Q-1153. Zo Brown Speaks, LLC showed interest in response to Sources Sought 36C24218Q0026 to FBO.gov, however, it was determined they did not provide capability to perform. A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: Planetree, Inc is the only firm capable of providing the services described above without the VA NJHCS experiencing duplication of costs that may not be expected to be recovered through competition and delays fulfilling its requirements.
