- Wage Determination Attachment 1 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, "Streamlined Procedures for Evaluation and Solicitation for Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. 2. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. 3. This is a combined synopsis/solicitation for Swine Blood Slaughter Collection (SBSC) Services as defined herein. The government intends to award a fixed hourly rate service contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. This procurement is set-aside for small businesses only. The associated North American Industrial Classification System (NAICS) code for this procurement is 115210, with a small business size standard of $7.5 million. 4. Requirement Scope The Pseudorabies disease surveillance collects blood samples from targeted populations of animals. Work schedule is inconsistent- will work 3-5 days per week depending on the operation schedule of the plant - work hours are typically start time of 5:30 am until 2:00-5:00 pm. The Contractor will work a flexible schedule, but no more than 90 hours per two week period. Work will be conducted at Tyson Foods, Inc, Slaughter Facility at 2000 Biffle Road, Newburn, TN 38059. ** SEE ATTACHMNET 1 FOR MORE INFORMATION ON WORK REQUIREMENTS AND QUALIFICATIONS REQUIREMENTS 5. Award shall be made to the responsible quoter based on best value in accordance with FAR Part 13. **PLEASE SEE FAR CLAUSE 52.212-2 ON ATTACHMENT 1 FOR EVALUATION CRITERIAS 6. Interested vendors shall submit: A. Completed Pricing Schedule with signature ( Page 5 of Attachment 1) B. Past Performance References with previous contracts/POs with this agency or similar scope of work with federal/state/local entities. Up to three past performance references or contract/PO numbers should be provided, if available. C. A technical statement explaining the proposed swine blood slaughter collection services offered. The statement should explain the firm's capabilities and how the offeror intends to meet the requirements discussed in the statement of work and the minimum qualifications attached below. D. 1- 3 resumes of individuals who can perform requested services 7. Clauses and Provisions that apply to this solicitation can be found in ATTACHMENT 1 8. Submission shall be EMAILED ONLY to Kimyen Tu, the Contract Specialist, at kimyen.n.tu@aphis.usda.gov. Quotes are due no later than March 16th, 2018 @ 1:00pm central time. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212‐1(f).
