Sources Sought # 36C26018Q9069 This is a SOURCES SOUGHT NOTICE for market research purposes only to determine the availability of potential businesses with capabilities to provide the service described below.     Service:   Laser Services for the Department of Veterans Affairs (VA), Puget Sound Healthcare System, Seattle, WA.   Provide rental of an ESWL/Lithotripsy, Holmium, Thulium, Omniguide, CO2, and/or KTP Laser machine and a certified technician to run the equipment and to provide technical support during the procedure.   Equipment will be used in the Surgery Suites.   Services are required on an as-needed basis, approximately one to 4 times per month for an estimated 173 cases per year.   The contractor s technical personnel shall be certified, trained, and current in all forms of laser procedure requested and in the operation of the equipment units.   The technician shall assist the attending physicians in the operating room in the operation of the equipment units.   The technician may be required to operate the device and any related imaging equipment during the procedures.   Contractor shall deliver all reports, digital, film and file records (if any) of all procedures performed with the devices to the operating room charge nurse in attendance.   These medical records shall be the property of the Department of Veterans Affairs. The preliminary intended contract period is 5 years or a one-year base period plus (4) four one-year option periods. Other ______ 3) If your company were selected, how many calendar days would your company need after the contract award date before you could be completely set-up and begin providing services?   15 days         30 days         Other ______ 4) How are Laser Services typically priced/how does your firm price Laser Services?   flat fee          Hourly          Other ______ 5) Provide a brief capability statement (less than 5 pages) with enough information to determine if your company can meet the requirement.   The Capabilities Statement for this Sources Sought is not expected to be a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding your company s ability to provide the service listed above.   Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement.   Provide any additional feedback that you feel is relevant.   Submission of a capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards.   The capabilities will be evaluated for determining to Set-Aside for the Small Business (SB) community or to conduct as an Unrestricted procurement.    Other than small businesses are encouraged to respond to this notice. This Sources Sought notice is for information and planning purposes only and is not to be construed as a commitment by the Government.   The Government will not pay for information solicited.
