- TITLE: FY'18 SY 19' 2HSJ - PATH SELF-MAILER - 2 LOTS (CONTROL AND UNIVERSAL GENDER) PRODUCT: 2 Lots Each are 10 panel, 5 leaved Self Mailer printed in 4 color process, Pantone 296, Pantone 187 and matte aqueous coating overall. Perforate in two places for BRC Cards, score and fold to a finished size of 7-1/8 x 5. Mailers are to be sealed with two (2) clear (minimum 1" in size), tab seals (middle-top and open left side end), must meet postal regulations. Jacket 504-682 Page 5 of 8 QUANTITY: 1,155,000 (No Overs/No Unders) BREAKDOWN: Lot 1 - Control - 920,000 (SJCA8) Lot 2 - Universal Gender - 235,000 (SJGA8) PLUS: 50 Quality Assurance Samples of Each Lot 12 Department Samples of Each Lot 1 Sample of Each Lot FORMAT: 2 Lots with different images. The self-mailers print in 4 color process, plus Pantone 296 (Blue) and Pantone 187 (Red) plus Matte aqueous coating. Follow laser prints for color breaks. All images are high resolution files. Follow files for color breaks and templates. MARGINS: Per art - face and back bleed 4 sides PERFORATION: Perforate two (2) 5" vertical blind perforation. One on the fold and the second between the 2 reply cards @ at 3-1/2" on the 5th panel. STOCK: JCP (L50) Matte Coated Cover, White, 80 Lb. TRIM SIZE: Flat - 35 -1/4" x 5"; Folded - 7-1/8 x 5" SCORE: Score and Fold - The 5th panel (reply cards) folds into the 4th panel (7"); then the 4th panel and 1st panel gatefold to the center and fold again. INK: 4 color process, Pantone 296 (Blue) and Pantone 187 (Red) + Matte aqueous coating overall.. FOLD: Panel 5 (BRM cards) folds into Panel 4, then Panel 4 and Panel 1 gate-fold to the center, and fold again to create a 7-1/8 x 5 finish size. Face panel ("HONOR - COURAGE - COMMITMENT") to be face up. Affix 2 (minimum 1") clear non-perforated wafer seals when looking at the address side, the wafer seals should be affixed to the top and open side. PACKING: Self-mailers are to be packed in cartons by code and shipped via truck. Pack in new corrugated fiberboard boxes. Do not shrink wrap. Use GPO 905 white shipping labels. Follow supplied sample. Carton labels must show title, item, quantity, source codes, form number and control number and must appear on the outside of the shipping carton. PALLET REQUIREMENTS: Pallets used for delivery to the destination listed below MUST be wood and have front and rear load beam, with a single (1) 48" long 2x4 upright support, centered on the length of the 48" dimension; this will allow a "fixed-fork unloader/uploader" to enter the pallets and place them in the warehouse racks. Pallets MUST be partially shrink-wrapped, and the size MUST be 48" x 42", with a maximum height of 48". Cartons MUST be stacked on pallets in a staggered manner to ensure maximum stability. NOTE: Pallets MUST be in accordance with the "pallet requirements" as described above and in the Iron Mountain Receiving Guidelines. Therefore, there may be cause for incidental expenses at the receiving warehouse that MUST be borne by the contractor. LABELING AND MARKING: (Package and/or Container label): Reproduce shipping container label from furnished repro, fill in appropriate blanks and attach to shipping containers. SCHEDULE: Unscheduled material such as shipping documents, receipts or instructions, delivery lists, labels, etc., will be furnished with the order or shortly thereafter. In the event such information is not received in due time, the contractor will not be relieved of any responsibility in meeting the shipping schedule because of failure to request such information. Furnished material will be available for pick up at the G.P.O., Southampton, PA 18966, on February 26, 2018.
