Page 1 of 1 The Department of Veteran Affairs, Iowa City, IA, intends to negotiate a sole source procurement under the authority FAR 6.302-3 with the University of Iowa for the period of 10/1/2017 to 9/30/2018 for work to facilitate development of an easy-to-follow step-by-step guide and visual materials (the virtual brown bag intervention) to help Veterans navigate the complex medication reconciliation process, and evaluate the usability and effectiveness of the intervention. Usability and effectiveness of the intervention will be assessed by 1) Veteran completion of the best possible medication list with and without assistance from a coach; 2) total number of PACT Nurse Case Manager reconciliations with and without the assistance of a study coach; 3) concordance between the best possible medication list and the CPRS medication list abstracted from CPRS four weeks after patient generation of the list; and 4) potential for adverse drug events and therapeutic duplication in the final medication list provided by the patient as compared with the pre-intervention medication list. VA Office of Rural Health project entitled: Virtual Brown Bag Intervention for Dual Use Rural Veterans led by Carolyn Turvey, PhD.
