Description

(i) In accordance with FAR Part 13, this combined synopsis/solicitation for commercial items is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, Southeast Louisiana Veterans Health Care System New Orleans has the need to procure Surgical Tower. (ii) The combined synopsis solicitation number is 36C25618Q0303. It s issued as a Request for Quotation (RFQ). (iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2005-95 (01-19-2017). (iv) The combined synopsis solicitation is set-aside for Veteran Owned Small Businesses (VOSB) only. The NAICS is 562119. Business size standard is 1000 employees. (v) This combined synopsis solicitation is for the provision of removal of grease and food waste from the grease traps located at the Canteen Service Kitchen located on the first floor of Building F and the Food Service Kitchen located on the fourth floor of Building G of the Southeast Louisiana Veterans Healthcare Facility, 2400 Canal Street, New Orleans, LA 70119. Responses shall be on an all or none bases, no partial submission. The Government reserves the right to make no award at all. See Attachment A: Price/Cost Schedule (vi) A description of the service to be acquired, and/or applicable Government need(s) as follows: The contractor shall provide all resources necessary to accomplish the deliverables described in the Performance Work Statement (PWS), except as may otherwise be specified. See Attachment: B Performance Work Statement (vii) The overall purpose is to provide service maintenance in accordance with the Performance Work Statement on the grease traps located at the Southeast Louisiana Veterans Health Care System (SLVHCS), 2400 Canal Street, New Orleans, LA 70119. See Attachment: C Contract Clauses and Provisions (xiv) Quotes are due on/or before March 23, 2018 at 2:00 PM CST by e-mail only at iris.ramirez@va.gov. Offers received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition or are deemed to be in the best interest of the Government. Note: Vendor Questions: All questions regarding this RFQ shall be submitted in writing and sent via email to iris.ramirez@va.gov no later than February 21, 2018 by 2:00PM CST. Site Visit Grease Trap Maintenance A one-time site visit will be conducted February 27, 2018 beginning at 9:00 a.m. CST, 2400 Canal Street, New Orleans, LA 70119. The site visit will begin promptly at 9:00 a.m. followed by a tour of the project site. Please respond to iris.ramirez@va.gov no later than February 23, 2018 2:00 PM CST with the name and email address of the person attending the site visit. All questions from the site visit shall be submitted in writing to iris.ramirez@va.gov.
