- This is a combined synopsis/solicitation for a twelve month maintenance agreement for nine (9) copiers at the Morgantown, WV, NIOSH Facility prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is OOHCCA16-2018-20371 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable: Maintenance Agreement for nine (9) copiers, Model #, Serial Number and copies required: CLIN 0001: RICOH/Aficio 3035 copier, Serial Number K9162500019. STATEMENT OF WORK Introduction/Background The NIOSH Morgantown facility located at 1095 Willowdale Road, Morgantown, WV, 26505 is a combination office and research lab facility. Objectives The objective of this project is to provide maintenance / service for the copiers throughout the facility. Scope of Work The contractor will repair or replace any part of the equipment which becomes unserviceable due to normal usage. Replacement parts to be furnished on an exchange basis. Services will be provided at the NIOSH facility during regular business hours, Monday through Friday, 8:00 AM - 4:30 PM. Preventative maintenance will be provided as determined by the contractor. Remedial maintenance will be provided after notification by the customer. Within one hour of receiving a request for remedial maintenance, the contractor shall provide the onsite NIOSH representative with an anticipated arrival time at the NIOSH facility. Equipment Equipment to be included under this agreement: • RICOH/AFICIO 3035 • RICOH/AFICIO MP 7503SP • RICOH/AFICIO MP C3501G • RICOH/AFICIO MP 6000 • RICOH/AFICIO 7001 (2 each) • RICOH/AFICIO MP 7502SP (3 each) Facility Access Contractor personnel will be required to comply with all NIOSH Morgantown security protocols in effect. Facility access, personnel identification badges, and parking tags must be coordinated through the security office at the L-Building entrance. The contractor shall be required to sign-in and out with NIOSH Security at the security entrance. All contractor personnel are required to present valid personal photo identification for the facility visitor sign-in procedure. All contractor vehicles are subject to search by CDC Security. Vehicles containing prohibited items will not be allowed to enter the NIOSH Morgantown facility and the contractor shall be directed to leave and shall schedule a return site visit as necessary to perform the service. Estimated Period of Performance: 04/01/2018 through 3/31/2019 Total ______________ The quote format is at the discretion of the offeror. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-66 (February 28, 2013). This acquisition is under North American Industry Classification System (NAICS) code 454390. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror.
