- Appendix A Fillable Records Verification Form Application and Approval Process R6 Records Verification MOU example Synopsis for Records Verification Provider Memorandum of Understanding The purpose of the Memorandum of Understanding(s) (MOUs) will be to provide a general framework for verification of all wildland fire personnel qualifications under Region 6 US Forest Service VIPR Agreements, in positions that require training above RT-130, including engine personnel, fallers, fire-line qualified EMTs and Paramedics. Under the terms of this MOU, members of Pacific Northwest Wildfire Coordination Group (PNWCG) and the Provider propose to form a partnership whereby the Provider agrees to verify personnel qualifications of contractors working under the Region 6 US Forest Service VIPR Agreements. According to the terms and conditions of this MOU, PNWCG agrees to recognize verification of wildland fire personnel completed by the Provider*. This will provide the checks and balances necessary for PNWCG to recognize that personnel qualifications meet minimum standards provided under the Region 6 US Forest Service VIPR Agreements. *Oregon Department of Forestry is the only agency that does not recognize the verifications provided by a 3rd party inspector for the Interagency Fire Crew Agreements. Providers will be notified of selection by March 5, 2018. Selected providers will be required to attend a 2-4 hour meeting at the Redmond Air Center, on March 14, 2018 to review what is expected of the providers.
