- Amendment 0002 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number for this effort is 12639518Q0059. This solicitation is issued as a Request for Quote (RFQ). SEE ATTACHED. (iii) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2005-97, effective January 24, 2018. (iv) This procurement is issued as a total small business set-aside. (v) Contract line item number(s) and items, quantities and units of measure: See attached. (vi) Description of requirements: Supply of insecticide strip. The solicitation will result in a five-year Indefinite-Delivery Indefinite-Quantity (IDIQ) type contract. (vii) Date(s) and place(s) of delivery and acceptance at FOB point: USDA APHIS PPQ, Warehouse Bldg 6420, Moore Air Base, 22675 N. Moorefield Road, Edinburg, TX 78541 (viii) The provision 52.212-1, Instructions to Offerors - Commercial, and Addendum apply. (ix) The provision 52.212-2, Evaluation - Commercial Items, applies. (x) The provision 52.212-3, Offeror Representations and Certifications - Commercial Items applies. (xi) The clauses 52.212-4, Contract Terms and Conditions - Commercial Items, and 52.212-4 Alt 1 apply. (xii) Clause 52.212-5, Contract Terms and Conditions Required To Implement Statues or Executive Orders - Commercial Items, applies to the acquisition. (xiii) Other contract requirement(s) or terms and conditions: See attached. Quoters are advised to carefully review the attached instructions for submitting a quote. A COMPLETE QUOTE SHALL INCLUDE THE FOLLOWING INFORMATION: 1) Cover page or Signed Standard Form (SF) 1449 and acknowledgement of Amendments to the solicitation. 2) Completed Pricing Schedule (Page 5) Provide a unit price for each line item as specified in the Pricing Schedule. Alternative pricing schedules will not be considered. Quotations must include a price for all items in the solicitation to be eligible for award. 3) Past Performance Information. Provide at least two (2) past performance references for recent and relevant work performed by the offeror within the last three years. Include project title, description, dollar value, dates performed, location, and contact information (name, title, valid email, and phone). 4) Product Information. (Do not submit if the brand name product specified is being quoted.) This is a brand name or equal procurement. Unless the quote clearly indicates that an "equal" product is being quoted, the quote shall be considered as offering the brand name product referenced in the solicitation. "Equal" products will be considered for award if the products are clearly identified and described by the quoter and are determined by the Contracting Officer to fully meet all of the salient characteristic requirements listed in the solicitation.
