Description
Please note that this is NOT a request for quotes or proposals. The Government is seeking Information for market research purposes only. The Department of Veterans Affairs, VISN 2, Network Contracting Office (NCO2) is currently conducting a market survey for firms capable of providing the following service: Rental of Digirad Cardius Multi-Head Cardiac Imaging System with N Speed. 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), specifically to provide one Digirad Cardius Multi-Head Cardiac Imaging System with N Speed. This system will be provided as a rental. The contract must reference that the hard drive remains at VA Hudson Valley and will never be returned to the vendor for any reason. 3. Background: Nuclear Medicine Systems have been in use at the VA Hudson Valley for over 30 years. This system has reached end of life and has experienced significant down time over the past three years. The project to replace the Nuclear Medicine System will involve a 12-week renovation that includes the installation of a SPECT CT System which will allow us to perform Nuclear Medicine exams faster and will also improve image quality. During this 12-week renovation/Installation period we plan to rent a Digirad Cardius Multi-Head Cardiac Imaging System with N Speed to provide veterans with Cardiac Nuclear Stress exams. 4. Performance Period: 3 months plus 3 months option. The system must be delivered by March 19, 2018. 5. Type of Contract: Frim fixed piece 6. Place of Performance: VA Hudson Valley HealthCare System Castle Point Campus 41 Castle Point Rd Wappingers Falls, NY 12590 Radiology Department ground floor. 7. Performance Based Contract: This contract requires the application of an Earned Value Management System that is compliant with the American National Standards Institute /Electronics Industries Alliance (ANSI/EIA) Standard-748, Earned Value Management Systems. [See paragraph Q.] B. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, together with associated submilestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed project management plan (PMP). The following Digital Radiographic/Fluoroscopic System and associated components are required; Desired System - Digirad Cardius Multi-Head Cardiac Imaging System with N Speed Dedicated Cardiology Nuclear Medicine System With Reading Station High Definition Solid State Solidium Detectors Count Head Acquisition Motion Correction Software Cardiac Reconstruction 4DMSpect cardiac Software Package SeeQuanta TrueACQ Count Based Imaging N Speed (Half Time Imaging) Advanced Software that features Workflow Tools Based Imaging 17 Display Monitor with Active Matrix Color LCD Screen Acquisition Software Small Foot Print Windows 7 Operating System 500GB Hard Drive 3GB RAM Staff Training Two Operators Manuals Two Service Manuals Set Up and On-site applications training Nvidia Geforce 8600M GPU Intel Core 2 Duo T9300 2.49GHz High Resolution 1920x1200 Monitor Acquisition and Processing Work Station Windows 7 Operating System Shipping to the VA Hudson Valley HCS, 41 Castle Point Road, Wappinger Falls, N.Y. 12590. VA Hudson Valley Retains System Hard Drive Containing PHI at End of Service 2. All written deliverables will be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms. 3. Where a written milestone deliverable is required in draft form, the HVHCS will complete their review of the draft deliverable within 7 calendar days from date of receipt. The contractor shall have 7 calendar days to deliver the final deliverable from date of receipt of the government s comments. The North American Industry Classification Systems (NAICS) is 811219 Responses to this notice must be submitted in writing via email and must be received not later than Monday, February 12, 2018 10 AM EST. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 811219. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities for which you have provided services to, proof of capably, certification. DUNS number, address, point of contact and social-economic category (ex:SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in System for Award Management (SAM), formerly CCR (see internet site: http://www.sam.gov). Responses to this notice must be submitted in writing, and received not later than Monday, February 12, 2018 10 AM EST.
