Description

Notice of Intent The Department of Veterans Affairs intends to negotiate a sole source commercial item procurement with Iridex Corporation. The Richmond Veterans Affairs Medical Center (VAMC) requests the following for their eye clinic: One each Cyclo G6 Glaucoma Laser Console MicropulseT Technology System with wired footswitch comprised of the following: One each power cord Two each keys Four each laser safety glasses If you are an authorized distributor of the above-mentioned items and your business can provide the exact requested items, please provide a capability statement as instructed below. Capability statements should include the following: Company/individual name Address Point of contact with phone number Information describing your interest Indicate FSS/GSA contract number or Open Market, if applicable Tax ID number DUNS number Capabilities Statement Company Socio-Economic SIZE (example large, small, SDVOSB, VOSB, WOSB etc.) Proof of verification as a SDVOSB/VOSB (if applicable) This proposed contract is for a G6 Glaucoma Laser Console System and MP3 probes for which the government intends to solicit and negotiate with only one source.   This notice of intent is not a request for competitive quotes; however, interested parties may identify their skill and ability to provide the exact requested equipment by sending capability statements in reference to this requirement no later than 12:00 p.m. noon EST, February 12, 2018 to:   desiree.mills-hicks@va.gov Respondents who submit proprietary data shall clearly mark the data with appropriate markings. The Government will not pay for any information submitted as a result of this notice. This notice shall not be construed as an obligation on the part of the Government to acquire any products or services. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government.   Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
