STATEMENT OF WORK (SOW) 501-18-2-107-0077 Contract Number: Task Order Number: IFCAP Tracking Number: Follow-on to Contract and Task Order Number: 1. Contracting Officer s Representative (COR). Name: Section: P&LMS, Administrative Office Address: 1501 San Pedro Se (113), Albuquerque, NM 87108 Phone Number: Fax Number: E-Mail Address: 2. Contract Title. Tabletop Cytocentrifuge 3. Background. The New Mexico VA Health Care System (NMVAHCS) Pathology and Laboratory Medicine Service (P&LMS) Chemistry Department requires 2 rapid spin tabletop cytocentrifuges for the quick processing of STAT plasma samples. 4. Scope. The Chemistry Department must process/centrifuge STAT plasma specimens in a rapid manner to meet testing turnaround expectations. Rapid spin centrifuges allow this spin time to be as little as 3 minutes which greatly improves the department s ability to begin testing on critical specimens form the Emergency Department and other critical care areas of the hospital. 5. Specific Tasks. We are requesting specific, high quality Brand Name or Equivalent to the following centrifuge: StatSpin 4 Centrifuge (Fisher HealthCare Catalog # 22 314 586) and the appropriate rotor (if purchased separately) to centrifuge 8 13 x 75 mm tubes Specific criteria the centrifuge must meet: One touch operation with three pre-programmed buttons for a 3-minute, 5-minute, or 10 minute spin time. Swinging buckets to allow for horizontal processing. Ability to produce platelet poor plasma in 3 minutes. Must accommodate 13 x 75 mm Vacutainer brand collection tubes Must be able to centrifuge at least 8 collection tubes at one time. Standard 110 V USA power compatibility. Benchtop footprint- no more than 30 lbs. 5.1 Task 1 - Enterprise Management Controls. 5.1.1 Subtask 1 - Integration Management Control. Use/lab maintenance of the centrifuges is described in detail in the chemistry procedure manuals. Deliverables: N/A 6. Performance Monitoring Lab staff will contact biomedical engineering if there are any questions about the performance of the centrifuge. 7. Security Requirements: The C&A requirements do not apply. A Security Accreditation Package is not required. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). N/A 9. Other Pertinent Information or Special Considerations. N/A a. Identification of Possible Follow-on Work. None b. Identification of Potential Conflicts of Interest (COI). N/A c. Identification of Non-Disclosure Requirements. None d. Packaging, Packing and Shipping Instructions. None, the warehouse can unpack the centrifuge. e. Inspection and Acceptance Criteria. The centrifuges should come with a warranty and will be inspected by Bio-med prior to use. 10. Risk Control Staff is familiar with use of centrifuges, which is part of annual safety training for all staff working in the laboratory. 11. Place of Performance. Govenrment site: NMVAHCS, Pathology and Laboratory Medicine Service, Chemistry Department, Bldg. 41, Room BA115. 12. Period of Performance. ASAP this is a one-time equipment purchase 13.
