DeBakey, Houston VAMC has a requirement for the purchase of Coloplast Implants used during surgical procedures. This will be a Consignment Agreement whereas items used will be replenished as needed for a period of five years (Base + 4 option years). Items needed are listed below. PLEASE RESPOND TO THIS RFI IF YOU CAN PROVIDE SUPPLIES THAT CAN BE DETERMINED EXACT TO THE PRODUCTS LISTED IN THE TABLE IN THE BACKGROUND SECTION ABOVE. IT IS THE RESPONSIBILITY OF THE SUBMITTING VENDOR TO PROVIDE DETAILS ON THE PROPOSED EXACT PRODUCTS, TO INCLUDE MANUFACTURER NAME, PART NUMBER, AND DESCRIPTION AND ITEM SUPPORT.
