Description
STATEMENT OF WORK (SOW) COURIER SERVICE GENERAL: Contractor shall provide all labor, material, equipment, transportation, and certification/licensure for transportation of clinical specimens to various labs for testing as needed. The majority of the trips will be to places such as Red Cross for special blood bank procedures. SERVICE LOCATION: VA Hudson Valley Health Care System PERFORMANCE PERIOD: Base Year Plus Four One-Year Options DESCRIPTION: A courier service is required to pick-up and to deliver clinical specimens to various labs including to St Luke s at Mount Sinai, Long Island City and other labs as needed. The majority of the trips will be for special blood bank procedures; therefore, courier service needs to be available anytime day or night and 365 days a year. VA Hudson Valley Healthcare System requires the courier to be at the VA Medical Center for pick-up within 30 minutes from the time a service call is placed. A dependable source is essential and for this reason, the contractor must have courier services within close proximity and be readily available to respond within the above timeframe as this is instrumental in times that urgency warrants special delivery. The delivery price schedule also includes various destinations; therefore, the courier service can be utilized for a comprehensive list of trips as needed for the laboratory. The number of trips is an estimated test count based on past 6 months, which are 252 trips to St Luke s at Mount Sinai for night shift stats (laboratory tests and services needed immediately) and one Blood Bank trip to Long Island City (ID lab for BB workups). Service will be requested on as needed basis. RESPONSIBILITY OF CONTRACTOR: The contractor shall maintain all Federal and State requirements/licensure as required. Medical specimens in transport shall be safeguarded according to Center for Disease Control and Prevention and Prevention and Dept. of Transportation regulations governing shipments of diagnostic/infectious substances. All drivers shall have specific training/supplies to handle cleanup of spills (Hazmat) etc. associated with medical specimens. All couriers must be eligible to enter VA Hudson Valley Healthcare Medical Facilities. Driver personnel records with documentation of required training will be furnished to the Contracting Officer Representative (COR) within 14 days of request. RESPONSIBILITY OF VA HUDSON VALLEY HEALTHCARE SYSTEM Provide originating caller name, contract phone number and location (i.e. laboratory, pharmacy, operating room etc.) to ensure packages are ready for immediate pick up. Provide all packing materials including shipping containers and ensure that all packages are correctly addressed. Ensure all packages are prepared or packed according to any Federal, State, and local regulations for diagnostic or infectious substances as appropriated to the contained specimens, etc. Ensure sufficient amounts of wet or dry ice are packed with the specimens if required to provide the proper storage temperature for the duration of the transportation process. Provide a contact name/number for both the sender and receiver. Be available 24 hours/7 days a week to answer technical questions assist with questions concerning the nature of specimens shipped. Coordinate access to VA Hudson Healthcare Medical Facilities according to current Security Forces rules, regulations and requirements. QUALITY CONTROL The contractor shall develop and implement a plan to monitor the quality and timeliness of services provided under this SOW. SAFETY The contractor and/or representatives shall comply with VA Hudson Valley Healthcare System safety standards at all times. Wearing seatbelts/shoulder harness and observing posted base traffic regulations and maximum speed limits while driving on VA Hudson Valley Healthcare grounds. Refrain from using cellular telephones while operating a motor vehicle on VA Hudson Valley Healthcare grounds. WORK LOAD ESTIMATE Services will be requested on an as needed basis. The number of estimated trips based on past six months is 504 per year to St Luke s at Mount Sinai for night shift stats and two Blood Bank trips to Long Island City.
