Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone, other Small and Large Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email of point of contact (e) DUNS number (f) Cage Code (g) Tax ID Number (h) If respondent is VOSB or SDVOSB, respondent is to provide proof of www.vetbiz.gov certification. In addition, SDVOSB/VOSB Contractors are asked to acknowledge that they understand the limitations on sub-contracting pursuant to FAR 52.219-14, Limitations on Sub-Contracting, which will be included in the impending solicitation. Furthermore SDVOSB/VOSB contractors are asked to respond that they are capable of providing the requested services keeping within the parameters of this clause (i)If an Offeror proposes to use any subcontractors, joint ventures, or teaming arrangements the Offeror shall include the percentage and type of work the subcontractor will accomplish to include their certifications. (Any subcontractors, joint ventures, or teaming partners shall have the required certifications as described in the sources sought notice) and (j) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. DRAFT STATEMENT OF WORK VISN 22 Gilbert Health Care facilities located in Arizona and New Mexico require Cardiopulmonary Perfusion services during open-heart surgeries on-site at the Department of Veterans Affairs Medical Centers. The contractor shall provide all resources necessary to accomplish the services. Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Facility Director. Each person assigned to work under this contract shall be licensed by the American Board of Cardiovascular Perfusion. The services include coronary artery bypass graft, valve replacement or ventricular assist devices. Services also consist of insertion of and monitoring aortic balloon pump, monitoring patients during PTCA, and Valvuloplasty. Indicate the perfusion technique you perform. Indicate the estimated volume of services you are able to support in the following areas: Albuquerque, NM, Phoenix, AZ, Tucson, AZ. A. Services shall be provided at a rate of at least one Perfusionist per surgical case. Open-heart cases are primarily performed during normal business hours, Monday-Friday (8:00AM 4:30PM), but the services are to be available on an emergency or urgent basis from the contractor at any required time, twenty-four hours per day, seven days per week. The contractor must be able to set-up the heart lung machine within one hour after notification of the need for services. B. The contractor, as machine operator, shall be responsible for the proper functioning of the heart/lung machine, the intra-aortic balloon pump, ventricular assist device (VAD), and any other devices necessary for safe cardiopulmonary bypasses. This includes set-up, calibration and safety check and other services considered to be operator maintenance. C. As part of the services provided, the contractor shall be responsible for assuring proper perfusion supplies are present in a sufficient quantity prior to beginning the case. Disposables shall be purchased by VA. D. Cardiopulmonary Standby requires the actual physical presence in the VA operating room, completely prepared, ready and capable to perform required perfusion services. E. If during a CABG, a right or left ventricular assist is required during procedure, a second perfusionist shall be provided by the Contractor. G. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws of the VA facility. The Contractor is subject to the provisions of the Privacy Act of 1974 and the Health Insurance Portability and Accountability Act (HIPAA) as well as all applicable VA laws and regulations. Contractor shall be limited in their access to only those VISTA menus required to perform the service. Contract employees will provide the Contracting Officer on an annual basis documentation of privacy training Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. If response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. The North American Classification System (NAICS) code for this acquisition is 622110 General Medical and Surgical Hospitals ($38.5). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete your Representations and Certifications (Reps and Certs). After review of the responses to this announcement, the Government intends to proceed with the acquisition and a subsequent solicitation will be published.
