- Attachment 1 Special Packaging Instruction Attachment 2 Schedule of Supplies/Services Combined Synopsis and Solicitation SP3300-18-Q-0027 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 - Streamlines Procedures for Evaluation and Solicitation for Commercial Items in conjunction with the Federal Acquisition Regulations (FAR) Part 13 Simplified Acquisition Procedures. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the solicitation number is SP3300-18-Q-0027 (see attached). 2. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-96 effective 06 November 2017 and Defense Federal Acquisition Regulation Supplement (DFARS) DPN 20171228 edition and Defense Logistics Acquisition Directive (DLAD) Current to Revision 5 and PROCLT 2018-03. The complete text of any of the clauses and provisions may be accessed in full text at http://farsite.hill.af.mil. 3. This Request for Quotation (RFQ) is being issued to establish a firm-fixed price contract for 1,000outer fiberboard boxes, inner fiberboard boxes, & foam insert in accordance with Special Packaging Instruction (SPI), Attachment 1 - SPI. AM13028723 dated 150527. The Schedule of Supplies/Services is provided asAttachment 2 - Schedule of Supplies/Services. Any award resulting from this RFQ will be issued on a Standard Form (SF) 1449. All dimensions are "inner dimensions" in inches. 4. ALL UNITS PRICES SHALL BE QUOTED AS FOB DESTINATION PRICING. 5. This RFQ will be awarded on a 100% small business set-aside. The North American Industry Classification System (NAICS) code for this project is 322211 and the size standard is 1,250 employees. Delivery Information: The items shall be shipped as follows: DODACC: SB3540 DLA DISTRIBUTION ANNISTON BOSS SUPP OFFICER 256-235-4119/4531 7 FRANKFORD AVE BLDG 362 ANNISTON AL 36201-4199 US 6. DELIVERY: Shipment of 1,000 completely assembled box (outer fiberboard box, inner fiberboard box, & foam insert) to be delivered within 60 days After Date of Contract.. 7. PACKING AND MARKING REQUIREMENTS. All documents must include at a minimum, contract number and document number for the item, stock number, quantity, and unit of issue. Omission of any data on the shipping documentation could cause a delay in receipt of goods and significantly delay payment. 8. Please pay special attention the Instruction to Offerors starting (page 14). The instructions require various items of documentation to submit with your quote; completed Representations and Certifications, etc. 9. The method of payment under this order is Wide Area Work Flow (WAWF). See DFARS clauses 252.232-7003 and 252.232-7006 below. 10. SYSTEM FOR AWARD MANAGEMENT - A prospective awardee shall be registered in the System for Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.sam.gov. 11. The Government is not soliciting for the "same old way" of doing business. The Government encourages proposals that will achieve savings through innovative processes. Furthermore, DLA Distribution personnel are working to eliminate unnecessary requirement and negotiating for a reduction in supplies/material prices. Therefore, your assistance is requested in reducing prices and improving our buying processes. 15. MARK CONTRACT NUMBER ON ALL CORRESPONDENCE Contractor must mark the contract of purchase order number on all paperwork and shipments. The order number must appear on the exterior of the shipment. Failure to follow these instructions will hold up payment to you and could result in the return of merchandise at your expense.
