- Community Care & HIMS IT Database Statement of Work Introduction/Background: The VA Central Western Massachusetts Healthcare System (VA CWM HCS) provides primary and secondary levels of medical care to a Veteran population in the five counties of central and western Massachusetts for more than 120,000 men and women. For services it is unable to provide in the main Northampton campus or the community-based outpatient clinics, the VA relies on area hospitals (community care providers). The transference of medical records and financial reimbursement data between the VA and these institutions is of paramount importance, but has become a cumbersome and labor intensive task. The VA CWM HCS refers approximately 6,500 consults for Veterans out to the community per year. There are about 60 community care facilities that treat these Veterans. Each of these facilities has different methods of sharing medical records and claims submission. The VA CWM HCS is seeking an IT contractor that can provide a subject matter expert (SME) to examine the current sharing processes, make recommendations for transference of records, and act as a liaison for local VA/Private Sector agreements and exchanges. Scope: The contractor shall be required to work with the Contracting Office Representative (COR), community care providers and the VA Office of Information and Technology (OI&T) to determine the best method of sharing medical records and claims submission as outlined below: Community care providers to fax to 413-582-3058 (Right Fax Machine) VA CWM HCS to connect to community care provider s Portal Mail paper records to VA CWM HCS (only if no other options are available) Upload of records onto disks, which are then mailed to VA CWM HCS Other possible solutions (discuss with COR) If a community care provider wishes for the VA to download records from a Secure File Transfer Protocol (SFTP) portal, a Memorandum of Understanding (MOU) will need to be drafted and submitted for the transference of medical records from the community care provider to the VA CWM HCS. The contractor shall assist the community care provider in the completion of the MOU template and then submit to OI&T. The VA expects approximately 10 MOUs will be needed the first year with an option to add more in subsequent years. The contractor shall assist the VA and community care providers with set up of these methods for receiving records from the community providers. The contractor will create a database of information containing at least the following information for each community care facility: Name of facility Address of facility Point of contact for facility (POC) in many cases this will be either within the Billing Department (Accounts Receivable) or Medical Records department of the facility Name, Department, Phone, E-mail Available methods of record sharing capabilities: Fax, Mail, Portal, Disk, etc. Preferred method for record sharing If MOU is needed for the portal, the following should be within the database MOU Identified Date MOU Completed Date MOU Submitted Date Performance Requirements: Review/identify current process and specifications of the VA s capabilities and determine which mode of retrieval is most beneficial to the VA and the community care provider. Identify best practices for transference of records. Outreach to community care providers/facilities Complete the MOU template with the community care providers/facilities, if needed. Implement solutions. Report out to VA Contracting Representative at least monthly, weekly is needed. Deliverables: Initial meeting with COR within 1 week of award of contract to discuss process, outcomes, and questions. MOU s shall be completed within 30 days of identification of the need for the MOU. Once completed, they will be sent to the COR and OI&T. Start with weekly meeting/call with COR and graduate to monthly meetings with the COR as determined by the needs of both the contractor and COR. Database of hospitals will be created and shared with the COR within 60 days of start of contract. Database will be shared with COR weekly as updates are completed via e-mail or Database or VA SharePoint.
