DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K) Acquisition Plan Action ID: VA250-18-AP-2558 Contracting Activity: Department of Veterans Affairs, VA Healthcare System, Network Contracting Office (NCO) 10, 3140 Governor s Place Blvd., Suite 210, Kettering, Ohio 45409. (2237) 506-18-1-4165-0001. Serving the Chillicothe VA Medical Center. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The contractor shall provide all labor, personnel, equipment, tools, materials, supervision and other items necessary to provide maintenance and software support on site for the NEC UM8700 located at the Chillicothe VAMC and as defined in this Statement of Work (SOW). Total estimated amount to include a Base Year and four Option Year periods is $52,500.00 Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Horizon Business Systems ( Horizon ) is a fully authorized NEC Corporation of America ( NEC ) Solutions Integrator in good standing. Horizon is authorized to sell and service NEC s UNIVERGE ® SV9100, UNIVERGE ® SV9300, UNIVERGE ® SV9500, UNIVERGE ® Blue Cloud Services and UNIVERGE ® 3C Products. Horizon s authorized territory is the entire State of Ohio. Description of market research conducted and results or statement why it was not conducted: Market research revealed no other business is authorized in performing the work as stated in the statement of work in the state of Ohio. NAICS codes 517311 was researched and revealed no small business, SDVOSB, or VOSB is authorized or capable of performing the work in the state of Ohio. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
