- Page 1 of 2 Request for Information (RFI) is issued solely for informational and planning purposes only. It does not constitute the issuance of a solicitation or a promise to issue a solicitation in the future. The Department of Veterans Affairs (VA), Veterans Integrated Service Network (VISN 8), is currently seeking information from any vendor that can provide service call diagnostic testing and repairs for High Tech Medical Equipment (Siemens Medical Solutions USA, Inc., Hologic Inc., Virtual Imaging, Inc., General Electric Co., Med Lab Supply and Canon Medical Systems USA, Inc.). Equipment repairs and services are at the below listed facilities: James A Haley Veterans Hospital and associated Clinics, 13000 Bruce B. Downs Blvd., Tampa, FL Orlando VA Medical Center and associated Clinics, 13800 Veterans Way, Orlando, FL VA Caribbean Healthcare System and associated Clinics, 10 Casia Street, San Juan, Puerto Rico Malcom Randall VAMC, NF/SGVHS and associated Clinics, 1601 S.W. Archer Road, Gainesville, FL Lake City VAMC, NF/SGVHS and associated Clinics, 619 S. Marion Avenue, Lake City, FL Miami VA Healthcare System and associated Clinics, 120 N.W. 16th St., Miami, FL West Palm Beach VAMC and associated Clinics, 7305 N. Military Trail, West Palm Beach, FL Bay Pines VA Healthcare System and associated Clinics 10000 Bay Pines Blvd, Bay Pines FL The applicable NAICS for this service is: 811219 other electronic and precision equipment repair and maintenance ($20.5 million SBA Size Standard). Vendors must be registered in the System for Award Management (SAM - https://www.sam.gov) prior to award. Any interested and capable firms are requested to provide the following information: Contractor must provide an authorized distributor letter for service/products from the Original Equipment Manufacturer to prevent gray market items. Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. (a) Gray market items are Original Equipment Manufacturers (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
