EE-133M-18-RQ-0091 A-0002 This amendment 0002 to the Request for Quotes (RFQ) EE-133M-18-RQ-0091 is to accomplish the following: A. Change this acquisition: From: 100% Small Business Set Aside To: Unrestricted/ Full and Open Competition. B. Extend the Solicitation/ Offer Due Date to January 12, 2018 at 5pm ET. C. Extend the questions due date in writing no later than January 11, 2018 at 12PM ET. D. All other terms and conditions remain the same. E. End of Amendment 0002 to EE-133M-18-RQ-0091. Amendment 0001 to the Request for Quotes (RFQ) EE-133M-18-RQ-0091 is to accomplish the following: A. *** In addition to the Original RFQ EE-133M-18RQ-0091 PAGE 17 AND 18 of the SF 1449*** 1.Vendors shall also provide OEM Distributor/ Service Authorization letter for the following equipment: a. Ansul b. Concelium c. Kidde d. Gaylord e. Fireboy- Xintex B. This Amendment is also to provide an updated Statement of Work (please see attached pages (3-17). C. All of the other terms and conditions of RFQ EE-133M-18-RQ-0091 remain the same. D. End of Amendment 0001. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This synopsis/solicitation EE133M18RQ0091 constitutes a Request for Quotation (RFQ) and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-96. The National Oceanic Atmospheric Administration (NOAA), Office of Marine and Aviation Operations, NOAA Ship Thomas Jefferson, currently located at the Marine Operation Center Atlantic 439 West York St. Norfolk, VA. 23510, requires Annual SCBA ABS Inspection and an ABS Annual Fire Inspection and repairs accordance with the attached Statement of Work. Place of Performance: NOAA SHIP THOMAS JEFFERSON 439 West York Street Norfolk, VA. 23510 Period of Performance: 01/16/2018 - 02/23/2018 Deliverables no later than: 03/07/2018 Government encourages a Pre-bid Inspection/ Ship check; however any Pre-bid inspection/ Ship check will be at vendors expense. Pre-bid Inspection/ Ship check will on: Tuesday 12/19/17 @ 9:00 am ET/ or Wednesday 12/20/2017 at 1:00pm ET or Thursday 12/21/2017 at 2:00pm ET. Perform Item 501 SCBA Servicing on NOAA Ship Thomas Jefferson in accordance with Detail Spec MOA2-01(18). ADDITIONAL ITEM REQUIREMENTS (AIR): These items relate to Growth Work. The rates quoted will be used throughout the life of the contract if growth work is added to the scope of the contract [reference Commerce Acquisition Regulation (CAR) clause 1352.271-72]. If AIRs are issued, these items will be awarded at the quantities needed via a contract modification. This RFQ is 100% set aside for small business concerns. The associated NAICS code is 336611, Ship Building and Repairing, with a business size standard of 1,250 employees. The Federal Supply Class is J020. In order to be considered for this award vendor shall provide the following information with your quote: 1. Proof of current liability insurance. 2. Complete name, address, telephone number and Duns and Bradstreet number. 3. Complete itemized quote that include ABS Fire Inspection and any additional repairs requested in accordance with the attached statement of work. 4. Include statement specifying the extent of agreement with all terms, conditions, and provision included in the RFQ. Quotes that rejects the terms and conditions of the RFQ may be excluded from consideration. 5. Proof of ABS Fire Extinguishing Systems Certification, at a minimum this must show current throughout the period of performance. 6. Proof of ABS self-Contained Breathing Apparatus Certification, at a minimum this must show current throughout the period of performance. 7. Provide available start date statement of when vendor can start work. 8. Acknowledgment of solicitation amendments (SF-10) if any. 9.
