Page 3 of 4 (i) This is a combined synopsis/solicitation for Automated Bag Sealer, BRAND NAME OR EQUAL, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. Simplified Acquisition Procedures IAW FAR 13 will be used. (ii) The solicitation number is 9291 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS 333993 code has a small business size standard of 500. The business has been verified for ownership and control pursuant to 38 CFR 74 and is so listed in the Vendor Information Pages database, (https://www.vip.vetbiz.gov); and The Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system such that the OEM warranty and service are provided and maintained by the OEM. All warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. The vendor shall provide new items ONLY; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Automated Bag Sealer shall be delivered to the Seattle VA Medical Center located at 1660 S. Columbian Way, Seattle, WA 98108. FOB Destination. Model #593003A2 (or similar model). Table-top baggers, meant for low volume packaging. Must bag minimum 20 bags per minute. All electric design does not require compressed air to run. Must accommodate bag size W2 to 11.5in. L 2 to 22in. Must have Automatic Bag indexing. One-touch sealer, easy change over.
