EMAIL INFORMATION TO : James.Vercoe@VA.GOV Statement of Work Anatomic Pathology Grossing Station 10/31/2017 Phoenix Veterans Administration Health Care System 650 E. Indian School Road Phoenix, AZ 85012 Transaction Number: 644-160-14-00014 POC: Claire Schmidt Histology Supervisor Pathology & Laboratory Medicine Service (P&LMS) Scope: The purpose of this requirement is to procure a new Grossing Stations for the Anatomic Pathology (AP) Department of the Pathology & Laboratory Medicine Service at the Carl T. Hayden VA Medical Center. This station will be in addition to the existing grossing station, which cannot accommodate current workload. This is an Accelerated Care Initiative-related proposal, since every anatomic pathology specimen is received at this station for examination, measurement, dissection, dictation, and preparation for processing. The supply acquisition does not involve connection of contractor-owned IT devices to the VA internal network, therefore the C&A requirements do not apply and a security accreditation package is not required. The equipment to be purchased does not have the ability to store any data. The equipment does not have the ability to store data or accept patient demographic information. Where will the equipment being requested be used: The grossing station will be installed gross room in the Histology Department of P&LMS. General Requirements for the Equipment: Specifications as set forth in this proposal are exact specifications and shall not be construed as limiting the overall quality, quantity, or performance of the equipment. The offerer shall meet or exceed the minimum requirements and shall be held responsible for the performance and overall quality of the requested equipment during the equipment s warranty period. The equipment, based upon the specifications requested, shall be the newest and the most current model. A refurbished is not acceptable. The equipment should be a standard product of the manufacturer of record. The manufacturer of record is defined as a company whose main occupation is the manufacture for sale items of equipment and which: Maintains a stock of replacement parts for the item. Maintains engineering plans, specifications, operating manuals & maintenance manuals. Has published and distributed literature and specifications on the equipment. Equipment Requirements for the Grossing Station: The equipment will fit within a footprint of 77.25 H x 72 W x 37.75 D. No length will exceed these limitations. The equipment will be free-standing and adjustable from a low position of 31.25 and maximum of 43.25 The equipment will have an alcove for a pathologist to sit in a chair and tuck his/her knees under the benchtop, and the alcove will be situated such that the pathologist can comfortably work at the cutting station. The side of the equipment where the sink is located does not need to have such an alcove. Sink will be on the Right side. The equipment will have utilities attachments for hot water, cold water, electricity, and exhaust/ventilation ported from the backside of the equipment. The equipment will provide fresh air supply on the cutting area and allow for complete fume evacuation. The equipment will have LED lighting contained within the cabinet of the grossing station. The equipment will have two GFCI outlet contained within the cabinet of the grossing station. The equipment will have a magnetic instrument bar for the grossing tools. The equipment will have a table flush system for easy clean-up. Any additional costs such as industrial funding fees, special shipping and handling fees, or other usage fees must be clearly indicated in the price quote. Optional Equipment Requirements for the Grossing Station: The following items can be optional features for the Grossing Station, so long as it is a compatible product manufactured with the expressed intent of being used with the Grossing Station. Availability and addition of each of these items will be considered to fully meet the requirements of this Statement of Work. Garbage Disposal 1 Hp Safety Splash Shield Cassette labeling shelf Computer CPU bracket Writing Ledge LCD Flat screen/keyboard support Formalin dispensing system Formalin collection system Pneumatic Foot pedal for formalin Gooseneck task light Magnifier for task light Installation, including Verification/Validation, Training: A technical support specialist from the instrument manufacturer will assist in equipment installation/set-up, providing all manpower, procedures, and supplies. The vendor/distributor and/or the manufacturer must provide all necessary procedure manuals, troubleshooting manuals and operator manuals. The vendor will provide primary user training for a minimum of one user (key operator) per instrument with on-site basic operating and maintenance education/training performed during initial installation. Any additional costs such as industrial funding fees, special shipping and handling fees, or other usage fees must be clearly indicated in the purchase price quote. All items quoted must be F.O.B. destination. All items shall be shipped to the following location (M-F, 8am-4pm, excluding holidays): Phoenix VA Health Care System Warehouse Receiving Dock 650 E. Indian School Road Phoenix, AZ 85012 Repair and Maintenance: Manufacturer will provide technical support by phone. Mandatory Value-Added Characteristics: These characteristics are mandatory, and will be considered in the best-value determination. Value added characteristics include, but are not limited to: Provide utility requirements (electrical, media disposal, plumbing, exhaust/ventilation, etc.) which should include all necessary SDS documents. Provide environmental requirements (heat/cool, humidity, light, air flow, etc.) and recommendations for instrument and testing material storage. The manufacturer should define maintenance requirements and indicate what maintenance is performed by the user and what is not. Support Services: The manufacturer should have the ability to provide phone service sufficient to meet the needs of the user facility Monday-Friday 8:00am to 4:30pm Arizona time.
