Description

NOTICE OF INTENT TO AWARD SOLE SOURCE CONTRACT: The Department of Veterans Affairs, Veterans Integrated Services Network 2 (VISN 2), hereby gives notice of their intent to award an interim sole source open market contract to CAREFUSION SOLUTIONS, LLC 3750 Torrey View Court San Diego, CA, 92130-2622. In accordance with the Federal Acquisition Regulation (FAR) 13 Simplified Acquisition. This sole source award will allow vendor to perform Service and Preventative Maintenance in accordance with the Statement of Work, to the Pyxis Equipment in the Hudson Valley Healthcare System at Montrose and Castle Point Campuses. STATEMENT OF WORK SERVICE/MAINTENANCE OF EQUIPMENT CareFusion/Pyxis I. EQUIPMENT: Equipment to be serviced and maintained includes: CareFusion Pyxis VA Entry # 40863 and all sub components purchased under 620-A30037 & A00014, ROOM 239-15H-CP, Montrose Pharmacy BLD 4 Ground floor and all seven CBOC s Equipment located at Montrose Campus to include: Med stations all with model number 3500 serial numbers 13074977, 13074981, 13074979, 13091154, 13139164, 8000047403, 8000047761, 8000047534, 800047533, 8000046454, 800046452, 8000046453, and 12532539. Also includes CII safe main serial number 8000046272..Equipment located at Castle Point Campus and seven CBOC s to include: Med stations all with model number 3500 serial numbers 8000047760, 12521797, 13360810, 13360811, 12556685, 12556688, 12556689, 12556690, 12556691, 12556692, 8000047402, 8000060864, 8000046448, 8000046449, 8000046450, 800046451, 125566 and 12556687. Also includes CII safe main serial number 8000046271. Smart remote managers at our seven CBOC s with serial numbers: 12598506, 12598509, 12598513, 12598515, Place of Performance: Equipment located in the Hudson Valley Health Care System and all associated CBOC s. The CareFusion Pyxis medication automation system is in use in 15CD, 4AB, 4CD, 6AB, 6CD, Domiciliary 28, Domiciliary 52, Montrose Urgent Room, Ambulatory Surgery, GU Clinic, B1, H-1, Chemo suite, Specialty, E-2, Castle Point Urgent Room, Goshen CBOC, New City CBOC, Pine Plains CBOC, Port Jervis CBOC, Poughkeepsie CBOC, Monticello CBOC and Carmel CBOC Performance Period: BASE PERIOD: Date of Award (DOA) through and including September 30, 2018 OPTION YEAR 1: October 1, 2018 through and including September 30, 2019 OPTION YEAR 2: October 1, 2019 through and including September 30, 2020 OPTION YEAR 3: October 1, 2020 through and including September 30, 2021 OPTION YEAR 4: October 1, 2021 through and including September 30, 2022
