Indian School Road Phoenix, AZ 85012 Equip: 1.0 Scope of Work (Purpose) The purpose of this performance work statement is to request the immediate purchase of (1) Combi-Oven/Steamer with Stand for Nutrition, Hospitality, and Food Service (NHFS). The contractor shall provide the equipment in accordance with this description of specification and other requirements of this request. The existing equipment safety standards are inoperable and has exceeded its 15year End of Life Cycle ; as well as, being deemed beyond economical value to repair and scheduled for turn-in as a result. Without the combi-oven/steamer, the department will be unable to prepare breads and varies food items ranging from roasting vegetables/meats to baking/proofing scratch bake goods; which will significantly impact NHFS ability/efficiency to provide the foods required for nutritional value for our veteran s patient care needs This system is essential for both assuring food quality and food safety/Hazard Analysis Critical Control Points (HACCP) quality compliance in food service. In accordance with Appendix A, Handbook 6500.6 Checklist block 6, the C&A requirements do not apply to this requirement, and Security Accreditation Package is not required. Indian School Road, Room 1153, Phoenix, AZ 85012. 4.0 System Documentation: 4.1.1 The contractor s proposal shall provide a complete set of unabridged operator manuals, service manuals, electronic schematics, troubleshooting guides, and parts lists for each piece of equipment which is part of the system procured by the MC. 4.1.2 The contractor s proposal shall provide a complete set of manufacturer s cleaning guidelines/instruction for each system component that is used. 5.0 Warranty: The contractor shall provide the standard 12 or 24 months warranty; with an option to purchase an extended warranty of 3-5 years that will cover all parts, labor, supplies, travel costs, and user support at no additional cost to PVAHCS. 6.0 Period and Place of Performance: The system must ship within 30 days as this is an essential piece of equipment for the hospital. (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies.
