- List of Ortho Supplies IEE Self Certification SF-18 RFQ This is a combined synopsis/soloicitation for various orthopedic medical supplies conducted under Simplified Acquisition Procedures (SAP) pursuant to the authority of FAR Subpart 13. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The current solicitation number is: 18-239-SOL-00014 and is issued as a Request for Quotation (RFQ). The clauses and provisions referenced in this solicitation may be obtained in full text form at http:/www.arnet.gov/far. The Department of Health and Human Services/Indian Health Service has a requirement for various orthopedic medical supplies. There is a list of orthopedic medical supplies as an attachment to this solicitation for the Bemidji Area Indian Health Service. Delivery/Ship to location: The Red Lake Indian Hospital, located in Red Lake, Minnesota. POC for this buy is Christopher.Millard@IHS.gov This is a 100% Indian Owned Small Business Set aside Basis for award: There will only be one (1) award for this solicitation resulting from the responsible offerors whose proposal represents lowest acceptable cost. Proposals shall contain the offerors best pricing. The Government intends to evaluate and award with no discussion. The Government reserves the right to make no awards as a result of this solicitation The associated North American Industrial Classification System (NAICS) code for this procurement is 339113 with the size standard of 750. **All proposals are to be submitted via e-mail to Christopher.Millard@IHS.gov.** The offeror should confirm receipt of e-mailed submissions.
