ANTICIPATED PERIOD OF PERFORMANCE: Base Year: 04/01/2018 to 03/31/2019 Option Year I: 04/01/2019 to 03/31/2020 Option Year II: 04/01/2020 to 03/31/2021 Option Year III: 04/01/2021 to 03/31/2022 Option Year IV: 04/01/2022 to 03/31/2023 COMPACTOR SERVICES: 1. Contractor to disconnect, pick up, empty, return, and reconnect VA owned 34 yard self-contained compactor, at least twice per week. 2. Contractor to provide emergency pickup service within four to six hours from time called by VA. 3. Specific days for service will be determined and adjusted with the approval of the Contracting Officer. The current schedule is Wednesdays and Saturdays. 4. Ticket to show pickup will be left with an EMS Supervisor after the compactor returns from the landfill. This ticket can be emailed to the VA Technical Representative no later than the following business day. 5. The compactor shall be cleaned and/or deodorized as appropriate or as needed. OPEN-TOP DUMPSTERS SERVICES: 1. The following open-top dumpsters shall be provided by the contractor. a. One 20-yard container for Research b. Two 30-yard construction containers c. One 20-yard wood container 2. The contractor shall pick up, empty, and return additional open-top dumpsters on an as needed basis. 3. Provide pick-up within 24 hours from time called by VA. 4. Provide emergency pickup service within four to six hours from time called by VA. RECYCLING SERVICES: 1. Contractor will provide 16 containers for comingled recycling and will ensure containers remain operational and clean/deodorized as needed. Containers must be under 96-gallon capacity. 2. Contractor is responsible for emptying all recycling containers contained in the recycling area at least once per week. 3. Cardboard bails will be picked up every other week, or if VA Technical Representative (or designated representative) calls for an emergency pick up. 4. The Contractor will provide, at a minimum, monthly weight and cost (and revenue) totals for each type of recyclable material (i.e. cardboard, paper, plastics, metal, etc.) removed from the facility. EQUIPMENT MAINTENANCE: 1. The Contractor shall provide to the VA Technical Representative a complete list of all equipment used in the performance of this contract with the name of the company and the capacity of the container in cubic yards within 15 days after contract award. 2. All equipment necessary for the performance of the service under this contract must be available for inspection prior to commencement of the contract and ready for use at the designated Government locations on the first day of service. 3. The Contractor shall take all necessary steps to ensure that conditions which may contribute to rodent infestation, such as accumulation of debris around and under trash equipment, unsanitary conditions and/or inadequately sealed/closed equipment are corrected as soon as they are observed or reported. The contractor shall correct reported deficiencies and report back to the VA Technical Representative within twenty-four hours after notification. PICK-UP TICKETS: 1. The Contractor shall issue pick-up tickets for conventional, compactor, and debris removal services. Pick-up tickets shall be prepared in duplicate by the contractor. It shall be the responsibility of the contractor to have all tickets numbered and dated the day of the pick-up. The ticket shall have the date and time of pick-up, the type of pick-up (trash or debris) and the signature of the contractor's employee performing the service. 2. The Contractor will provide a monthly listing of the weight (tons) and costs (i.e. pull charge and landfill charge) associated with each container or dumpster of general wastes removed from the facility. DISPOSAL FACILITY: Selection of a certified disposal facility shall be the responsibility of the contractor. SAFETY REQUIREMENTS: In an effort to better protect Government and Contractor personnel, the contractor shall ensure that their drivers follow all prudent safety practices while operating a motor vehicle. All large vehicles shall have operable parking and emergency parking systems, backup alarms, and wheel chocks. ENVIRONMENTAL QUALITY ASSURANCES: 1. Contractor shall fully comply with all state, county and city laws and regulations regarding sanitation and solid waste disposal. 2. Contractor shall turn off the motor of all vehicles that do not require the motor to run for-the-purpose of loading the vehicle. 3. Contractor shall not create any litter at loading locations during any transportation, and/or disposal locations, to include oil leaks and leakage from containers. 4. Contractor shall follow EPA regulations outlined in the Code of Federal Regulations 40 parts 241, 243 and 257. INVOICING: 1. The Contractor will provide a monthly listing of the weight (tons) and costs (i.e. pull charge and landfill charge) associated with each container or dumpster of general wastes removed from the facility. 2. The contractor shall send the Iowa City VA a monthly check for revenue of recycled items. This check should be written to Iowa City VA Medical Center, with Recycling Program in the memo and sent to the following address: Iowa City VA Medical Center Attn: Agent Cashier 601 Highway 6 West Iowa City, IA 52246
