Description

This is a REQUEST FOR INFORMATION (RFI) only pertaining to brand name or equal supplies for the Indy West VAMC, in Indianapolis, Indiana. Information collected during this RFI may be used in a set aside. If a solicitation is issued, the Government will do so in accordance with Federal Acquisition Circular (FAC) 2005-83. The North American Industry Classification System (NAICS) number is 339113. The NAICS size is 1000 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. Contractors that deem themselves capable of meeting the requirement shall provide the below information to John Saunders, Contracting Officer, at Royce.jones@va.gov no-later-than Wednesday, November 22, 2017 at 4:30PM, EDT, referencing (36C25018Q9044). Responses shall include: (1) Business Name and Address, (2) FSS Contract Number if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. Contractor must be registered with https://www.sam.gov Supplies are to include only TAA compliant items. Items that are not TAA compliant must be clearly identified by the contractor on the contractor s quote should a solicitation be subsequently issued. FOB is Destination. Delivery and installation is needed 30 days ARO. Delivery is to: Indy West VA Medical Center 3850 Shore Drive, Suite 203 Indianapolis, IN 46254 To be considered an equal the supplies being offered shall have the salient characteristics of the following: Scope The following criteria must be met in the final selection. Common to both rooms: Overall lower casework size to be no more than 19 deep, 5 long and 36 tall. Lower and upper cabinets to be constructed of powedercoated 18 gauge cold rolled steel, finished on all four sides with leveling screws on each corner. Color TBD Lower cabinets to consist of 2 sets of drawers, each at 24 wide, total of 48 wide. Upper 3 drawers at 4 tall and bottom drawer at 8 tall. Drawers to be one piece molded polystyrene body with rounded corners. Upper cabinet to be 48 wide x 24 tall. (2 @ 24 wide with single door each is acceptable). One left hand door, one right hand. Provide 2 interior adjustable shelves in upper cabinet(s) to be 18 gauge cold rolled steel. Color to match casework. Door/Drawer fronts to have 12 mil seamless vinyl wrap. Color TBD Hinges to be Blum concealed, self-closing, 110 degree opening, nickel plated metal, clip-on mount, three adjustable points. Door and Drawer handles to be brushed nickel pulls. Style TBD Upper cabinets to be mounted to wall using metal mounting brackets provided with cabinetry. Room 214: Countertop to be post form HPL 1-1/2 thick top with integrated backsplash at 1 thick by 3 tall. When mounted to the wall, the right end of the countertop is to have a 90 degree radiused end for safety. (see drawing below) Room 215: Countertop to be post form HPL 1-1/2 thick top with an intergrated backsplash at 1 thick by 3 tall. When mounted to the wall, the left end of the countertop is to have a 90 degree radiused end for safety. Right side to have matching sidesplash to protect adjoining wall. (see drawing below) Warranty: Lifetime warranty against defects of materials and workmanship under normal use and service. Minimum 5 year warranty.
