Kingsbridge Road Bronx NY 10468 Synopsis: Please note that this is not a request for quotes or proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, Northport VA Medical Center, 79 Middleville Road, Northport, NY, is conducting a market survey for qualified firms capable of providing the following services for Northport VAMC: The contractor shall collect all of VA Northport s chemo waste placed in 55-gallon drums on the loading dock of the hospital-Building 200. (Draft statement of work below). The North American Industry Classification Systems (NAICS) is 562119 with a size standard of $38.5 million. Responses to this notice must be submitting in writing via email and must be received by November 20, 2017. The purpose of this notice is to gain knowledge of potential qualified sources for this procurement relative to NAICS 562119. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, and examples of similar facilities which you have provided services to DUNS number, address, and point of contact. If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Responses to this notice must be submitted in writing (email) and received by November 20, 2017 at 12:00 PM EST. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.sam.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Email: marshand.boone@va.gov VA Northport Medical Center CHEMO WASTE REMOVAL DRAFT Statement of Work SCOPE Description of Program Chemo Waste Container Collection/Removal. Labor and materials necessary for the collection and removal of Chemo Waste Containers. The contractor will perform all required services identified in this statement of work (SOW). SPECIFICATIONS: The contractor shall collect all of VA Northport s chemo waste placed in 55-gallon drums on the loading dock of the hospital-Building 200. Pick-ups will be scheduled at a minimum of one (1) per week and as needed where applicable. If quantities increase; pick-ups shall be adjusted accordingly. Pick-ups will be made at the loading dock of Building 200 (Main Hospital) between 8 AM and 4:30 PM Monday through Friday. Contractor provided containers shall be leak-proof, stackable, sealable, transportable, and designed so that government employees will not be required to lift more than 35 lbs., and shall display the universal biohazard symbol. All containers shall be damage free and in good working order at all times. Containers must be identifiable as to the type of hazardous waste contained within, (i.e. yellow for chemo) all containers shall be approved by the COR. Contractor will remove full containers and replace them with an equal number of empty sanitized containers. Sanitizing shall be completed off of grounds. Prior to departing any/all locations listed within this SOW; contractor shall obtain the facility s designee s signature for confirmation of manifest including but not limited to: Pickup/Delivery Location, Pickup/Delivery Date, Amount of waste (number of containers picked up and/or pounds), Destruction Certification Document/Manifest Number and any other information as required by the COR. The Contractor shall transport and dispose of material in accordance with all applicable Environmental Protection Agency (EPA), Department of Transportation (DOT), Occupational Safety and Health Administration (OSHA), and any other state and federal guidelines. GENERAL: Location of Hospital: Northport VA Medical Center, 79 Middleville Road, Northport, NY 11768. Regulated Medical Waste (RMW) includes but is not limited to items contaminated with blood and body fluids/waste, chemotherapy wastes, pathological and microbiological waste or other potentially infectious materials. Work Hours: Normal Work Hours: (7:00 am to 9:00 pm) National Holidays: The ten holidays observed by the Federal Government are: New Year s Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. Period of Performance: The base year for this contract is January 1, 2018 through December 31, 2018. There will be four subsequent option years: Option Year 1 (January 1, 2019 December 31, 2019); Option Year 2 (January 1, 2020 December 31, 2020); Option Year 3 (January 1, 2021 December 31, 2021); and Option Year 4 (January 1, 2022 December 31, 2022). DOCUMENTATION FROM THE CONTRACTOR: The contractor shall prepare and submit to the COR an original signed Certificate of Destruction for each pick up for each site within ten (10) working days of each pickup. The contractor shall maintain proper records concerning each Certificate of Destruction issued. The Certificate of Destruction shall at a minimum contain the following: Pickup/Delivery Location, Pickup/Delivery Date, amount of waste (number of containers picked up and/or pounds), Destruction Certification Document/Manifest Number, Destruction Certification Date, any other information as required by the COR. TRAINING REQUIRED OF CONTRACTOR: Contractor shall provide annual training to government staff as required by the COR. Training shall include proper handling of medical/special waste; interim storage requirements, proper identification and labeling of wastes, etc. QUALITY ASSURANCE: The Government will investigate complaints. All information received will be provided either verbally and/or in writing to the contractor for appropriate action. The contractor shall maintain each facility to present a neat, clean and professional working environment at all times. Except for those items or services specifically stated to be government furnished, the contractor shall furnish and maintain everything required to perform this contract in accordance with all of its terms. Management of regulated medical waste in the workplace will follow OSHA 29 CFR 1910.1030. All trash will be collected and removed to a location designated by the customer. Chemo waste will be disposed off-site by contractor. Containers shall be sealed to prevent leakage and handled in a manner that will prevent bursting or tearing during transport. Liquid containers shall be placed in break-resistant and tightly sealed packages. All exterior containers used shall have the words "chemo waste" or "infectious waste" or display the biohazard signage. Chemo waste, as with all Regulated Medical Waste (RMW), must be segregated from general trash at the place where it is initially generated; the segregation must remain until final disposal has been accomplished. Never mix chemo waste with regular trash. Use yellow bags (or any puncture resistant, leak resistant, and uniquely colored or marked) to hold chemo waste.
