Description

This notice is for the purpose of seeking all capable socio-economic sources to provide Consultation, Evaluation and Development services. The VA New Jersey Healthcare System (NJHCS) has a requirement for an organization to consult, evaluate and develop a certification program based on the Planetree model in patient and person-centered care across the continuum of healthcare. The certification sought shall be universally recognized for the kind of structured, standards-based quality improvement framework that a healthcare program provides. A person-centered culture is not built solely around the patient experience and their needs and preferences, but must also address the experiences and needs of family members, as well as the healthcare professionals. Certification must be recognized by the Joint Commission and represent the highest level of achievement in patient/person-centered care based on key elements necessary to implement and sustain organizational cultures; establish person-centered excellence as a definable goal that can be set, tracked and achieved; provide a concrete framework for implementation; and provide external validation and recognition that differentiates healthcare organizations demonstrating superior levels of achievement in patient/person-centered care. The Planetree model for Patient/Person-Centered Certification builds upon a tiered system of recognition: Bronze for Meaningful Progress in Patient/Person Centered Care Silver for Significant Advancement in Patient/Person Centered Care Gold for Excellence In Person Centered Care In 2017, the NJHCS was awarded Silver Recognition for Significant Advancement in Patient/Person-Centered Care for the continuing care environment. They are now seeking to attain the third tier recognition of Gold for Excellence in Person Centered Care of the Planetree model. The performance period shall be for a base period with the possibility of four (4) option periods. Services are needed to commence on or about January 1, 2018, and the procurement is being conducted under FAR Part 12 Commercial Items and FAR Part 13.5 Simplified Acquisition Procedures. The NAICS code is 540611, and the size standard is $15 million. The solicitation package can be viewed or downloaded, at no charge, from the link http://www.FBO.gov when published. This solicitation package may be available on or about December 19, 2017. Response to this announcement shall determine if sufficient competition exists to set aside the procurement for Service Disabled Veteran Operated Small Business, Small Veteran Owned Business, or Small Business.
