52.212-3 Offeror Representations and Certifications - Commercial Items (Jan 2017) Solicitation Deviations DOJ Whistleblower Information Technical Quotation Form Business Quotation Information September 2017 SF 1449 Cover Letter Pittsburgh, PA The Federal Bureau of Prisons, Community Treatment Services (CTS) Contracting Office; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse, Mental Health and Sex Offender Treatment Services in the Pittsburgh, PA area. The location where services are to be provided should be with a vendor within a 5 mile distance from Pittsburgh City Hall located at 414 Grant Street, Pittsburgh, PA 15219. This requirement will ensure that the vendor is centrally located and within walking distance of public transit if necessary, and ensure inmates are not subjected to walking long distances in adverse weather. Performance periods under this contract will be for a one-year base period to begin on or about April 01, 2018 with nine (9) one-year option renewal periods with service ending (if all options are renewed) on March 31, 2028. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three, Option Year Four, Option Year Five, Option Year Six, Option Year Seven, Option Year Eight, and Option Year Nine, respectively. The following services are required and may be sub-contracted to another licensed contractor.
