Section J-4.E - Submission Documents WD_2015-5545, Rev 4, 7.25.17 PM Schedule 5850 rev 2012 Past Performance Questionnaire Contractor Information Worksheet (CIW) GSA850-6-28-2017 GSA Form 139 Record of Time of Arrival and Departure from Building Introduction Consolidated Facilities Management Service This solicitation contains a Consolidated Facilities Management (CFM) work statement for a performance-based service. The CFM approach combines facilities related contracts into a single contract designed to reduce administrative costs and simplify the management of various building services. The performance-based service means that the Government has described WHAT is to be accomplished, NOT HOW to accomplish it, and states a basis for determining whether finished work meets the Government quality requirements. It does not state detailed procedures for accomplishing the work unless there are safety, security or communication requirements. It is the responsibility of the Contractor to ensure that they build into their bid/proposal price, the necessary hours that it will take to meet the performance standards specified. The Contractor will be required to maintain a quality control program to ensure that the requirements of this contract are met. The Contractor shall provide the management, administrative, supervision, labor, sub-contractors, materials, supplies and equipment (except as otherwise provided), and shall plan, schedule, coordinate, and assure effective completion of Preventive Maintenance services, repair of building operating equipment and systems as described in this contract. The Contractor shall be responsible for needed engineering services and reimbursable building alterations of building equipment including, but not limited to: All elevators on the premises, custodial and trash/debris removal services and all miscellaneous work associated with complete building facility management at the building. The Contractor will determine how much labor is needed to perform the tasks, what methods will be used to complete the job and which supplies, materials and equipment are needed.
