General Objectives and Requirements Purpose: The Phoenix VA Health Care System (Agency) requires a repair-exchange contract to cover (7) GlideScope AVL batons. Authority for the Contract: 38 U.S. Code § 7301.b The primary function of the [Veterans Health] Administration is to provide a complete medical and hospital service for medical care and treatment of veterans, as provided in this title [Functions of Veterans Health Administration] and in regulations prescribed by the [Under ]Secretary [for Health] pursuant to this title. The Surgical Service uses Verathon GlideScope advanced video laryngoscopes (AVLs) to assist in opening the airways of patients. The baton Is susceptible to damage due to rough and heavy use. The batons are not repairable and can only be exchanged for a new, functional baton. Each replacement baton costs over $2000. The Agency will break even on the service contract after replacing (3) batons. The Agency currently has 7 GlideScope AVL batons and two are already broken. Scope. Coverage period: Monday-Friday, 7am-4pm LST, excluding federal holidays. The Contractor shall accept the broken baton and send the Agency a new, functional baton. The new baton shall be compatible with the Verathon GlideScope AVL. The cause of damage shall not be considered when covering the baton. The Contractor shall ship devices with next day delivery. The Contractor shall deliver a hard copy or electronic copy of a Field Service Report that delineates the service performed and confirms quality assurance tests results to Agency. The Contract shall provide for the addition and removal of equipment. Security Requirements. Security Requirements for Unclassified Information: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. The C&A requirements do not apply. A security accreditation package is not required. Performance Monitoring. The Agency shall oversee work done by the vendor and accept the quality of work as appropriate. Other Pertinent Information or Special Considerations. Packaging, Packing and Shipping Instructions: All parcels and packages shall come through the Agency s warehouse to be distributed either through Biomedical Engineering or the field service engineer her/himself if s/he is on station to receive the package. All parcels and packages shall ship overnight. Inspection and Acceptance Criteria: The Agency shall sign off on the work completed if service is determined to be satisfactory. Place of Performance.
