- NOTICE OF CONTRACTING OPPORTUNITY This proposed contract action is an Individual Set-Aside per DFARS 237.104(b)(ii)(B). It is a Department of Defense (DoD) contracting opportunity reserved solely for "individuals under the authority of 10 U.S.C. 1091." Specifically excluded from this opportunity are "business entities other than individuals." The Government intends to solicit and negotiate an "individual" to provide Phlebotomy Services at US Naval Hospital Guam. This will be a Firm-Fixed Price contract with possible option periods. All individuals responding to this notice must be registered with SAM (System Award Management) website www.sam.gov. This notice is for commercial services prepared in accordance with FAR Subpart 13.1. The NAICS Code is 541380 Testing Laboratories. (See attached "Required Documents and Scope of Work") SUBMISSION: All required documents shall be submitted electronically on or before the date due to: brianwilliam.c.mafnas.civ@mail.mil E-mail submissions are limited to 2MB. The submitter should confirm receipt of email submissions. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-75. It is the contractors' responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisitions.gov and or www.asq.osd.mil. 52.212-2 Evaluation - Commercial Items (JAN 1999). The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Evaluation factors in descending order of importance: 1. Technical Capability 2. Price The following factors to be used to evaluate offers: Technical Capability, defined as ability of the individual provider to meet the job qualification requirements and provide the services required. Technical capability is a significant evaluation factor that will be most advantageous to the Government.
