Description

The National Cemetery Administration has issued solicitation number 36C78618Q0004 as a Request for Quotation (RFQ). This is a Request for Quote for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a solicitation is attached. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-89. This combined synopsis/solicitation for commercial items is in accordance with Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition Procedures. This acquisition is 100% set aside for Service-Disabled, Veteran-Owned Small Businesses. The Contractor shall provide uniform and mat rentals for Fort Custer National Cemetery in accordance with the attached Statement of Work (SOW), to the following addresses: Fort Custer National Cemetery 15501 Dickman Road Augusta, MI 49012 BASE YEAR: October 1, 2017 September 30, 2018 CLIN ITEM EST QTY UNIT UNIT PRICE TOTAL PRICE 0001 Uniform rental and laundry service for 16 full time employees and 6 seasonal employees along with 6 floor mats in accordance with the attached Statement of Work 12 MO OPTION YEAR 1: October 1, 2018 September 30, 2019 CLIN ITEM EST QTY UNIT UNIT PRICE TOTAL PRICE 1001 Uniform rental and laundry service for 16 full time employees and 6 seasonal employees along with 6 floor mats in accordance with the attached Statement of Work 12 MO OPTION YEAR 2: October 1, 2019 September 30, 2020 CLIN ITEM EST QTY UNIT UNIT PRICE TOTAL PRICE 2001 Uniform rental and laundry service for 16 full time employees and 6 seasonal employees along with 6 floor mats in accordance with the attached Statement of Work 12 MO OPTION YEAR 3: October 1, 2020 September 30, 2021 CLIN ITEM EST QTY UNIT UNIT PRICE TOTAL PRICE 3001 Uniform rental and laundry service for 16 full time employees and 6 seasonal employees along with 6 floor mats in accordance with the attached Statement of Work 12 MO OPTION YEAR 4: October 1, 2021 September 30, 2022 CLIN ITEM EST QTY UNIT UNIT PRICE TOTAL PRICE 4001 Uniform rental and laundry service for 16 full time employees and 6 seasonal employees along with 6 floor mats in accordance with the attached Statement of Work 12 MO Questions pertaining to this solicitation must be sent by e-mail to: john.carlock@va.gov.
