- FA3016-17-Q-0003 LAK Fans SOW FA3016-17-Q-0003 LAK Fans Provisions & Clauses THIS IS A BRAND NAME OR EQUAL, REQUEST FOR QUOTE, SUBJECT TO AVAILABILITY OF FUNDS. Offerors shall respond to this RFQ by email response to: Ms. Barbara A. Jewett; E-mail: barbara.jewett.1@us.af.mil ; and Ms. Melody Taylor; Email: melody.taylor@us.af.mil ; not later than 10:00 am CT, 22 September 2017. Any quote (also referred to as 'offer'), modification, revision, or withdrawal of a quote received after the time specified above will be considered late and may result in the being ineligible for award. Joint Base San Antonio (JBSA) Lackland, has a requirement for Industrial Fans with Installation as described herein. •1. This requirement is a Total 100% Small Business set-aside. The applicable NAICS is 333413, Industrial and Commercial Fan and Blower and Air Purification Equipment Manufacturing, size standard is 500 employees. Only quotes submitted by small business concerns will be accepted by the Government. Any quote that is submitted by a contractor that is not a small business at the time of quote submission will not be eligible for award. •2. The Government intends to issue a single, fixed-price purchase order to the offeror providing the lowest price and meets technical acceptability requirements. •3. Basis for Award: Lowest Price Technically Acceptable (LPTA). Offerors shall submit enough technical literature on each CLIN as is necessary for the government to be able to clearly determine if the quote is for the Brand Name item(s), or if it meets the salient characteristics if offering an 'equal' item. •4. Mandatory Registrations. To be eligible for award, an offeror's registration with the System for Award Management (SAM) must be current. To register or update former CCR and Reps & Certs that are not current go to https://www.sam.gov/portal/public/SAM/ and provide mandatory information. •5. Site Visit. A site visit will be held on 19 September 2017, at 9:00 AM CT. This will be the only Site Visit held. All vendors will meet the Government POCs, listed above, at the JBSA-Lackland Visitor Center, at 9:00AM CT. Please DO NOT go directly to the site without the Government POCs. If interested in attending, please contact the above listed POCs and provide the following information: Name, DOB, Driver License Number and State of Issue, by 18 September 2017 at 2:00PM CT. •6. Delivery: Delivery shall be FOB Destination, 60 Days ARO with all associated costs included in the price of each CLIN.
