- Solicitation/RFQ SECTION A - Solicitation/Contract Form September 7, 2017 A.1 REQUEST FOR QUOTE From: Department of Health and Human Services Program Support Center Acquisition Management Services Subject: Request for Quote 17-233-SOL-00895: Xray and Scanner RA600 Equipment and Services The Department of Health and Human Services (HHS), Program Support Center (PSC) is issuing this Request for Quote (RFQ) for the purpose of entering into a contract with the selected Contractor. PSC will conduct this acquisition using Part 12 (Commercials Items) under the Federal Acquisition Regulation. This requirement is for the Defense POW/MIA Accounting Agency (DPAA), JBPPH Hawaii. The corresponding NAICS code is: 621512 "Diagnostic Imaging Centers". Evaluation Factor: is a Lowest Price Technically Acceptable (LPTA). This solicitation does not commit the Government to pay any costs for the preparation and submission of your proposal. It is also brought to your attention that the Contracting Officer is the only individual who can legally commit the Government to the expenditure of public funds in connection with the proposed requirement. QUOTE SUBMISSION Note that the closing date for this solicitation is 2:00 p.m. EST, Thursday, September 14, 2017. Questions will be accepted until 11:00 a.m., Monday, September 11, 2017. In accordance with far 52.212-1(f), quotes received after the established closing date will be viewed as late, and therefore not considered. All questions regarding this solicitation should be submitted in writing to Joy Penno at joy.penno@psc.hhs.gov.
