This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ-VA250-17-Q-1141 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95/ 01-19-2017. The North American Industry Classification System (NAICS) number is 339113 and the business size standard is 750 employees. STATEMENT OF NEED Endo Stratus Carts and Dual Yoke Brand Name or Equal to: ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT 0001 DUAL YOKE LOCAL STOCK NUMBER: EGA-7029 4.00 EA 0002 ENDO STRATUS Box Style Cart with Dual LOCAL STOCK NUMBER: EGA-7033 4.00 EA Salient Characteristics: Box Style Cart (Dimensions below) that will hold 2 CO2 tanks and the CO2 regulators as seen in picture. A yoke for each cart and tanks. Dimensions in picture. The proposed contract listed here is unrestricted. FOB destination. Deliver to VA Medical Center, 4646 John R. Street, Detroit, MI 48201.
