- 17-242-SOL-00076 - Zuni CT Scanner The Albuquerque Area Indian Health Service (AAIHS) has a requirement for the following supply/service: (15) BRAND NAME OR EQUAL to: Philips 100032 Ingenuity Core, that include NCTD269 Standard Table NCTA176 Operator's manual - English NCTA485 Keyboard Language - English NCTA131 Computer Table 989605200562 Teal 100kVA Isotran LM 989801292078 Full Travel package for Off-Site Training Any Trade-in Allowance: ** Philips Model # 728246 Brilliance CT - 16 slice, s/n: 6387 Delivery Location: Zuni Comprehensive Community Health Center Rout 301 North "B" Avenue Zuni, New Mexico 87327 Prospective Offerors are invited to submit proposals in reference to solicitation no.17-242-SOL-00076. The solicitation package is comprised of all information including the estimated requirements. The Government anticipates in awarding a single, fixed-priced requirements type contract for a one time purchase. Fax a. Fax to: (505) 256-6848 New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty and detail that time period. Quote MUST be good for 30 calendar days after close of solicitation. Shipping must be free on board (FOB) destination CONUS (Continental U.S.), which means that the seller must deliver the goods on its conveyance at the destination specified by the buyer, and the seller is responsible for the cost of shipping and risk of loss prior to actual delivery at the specified destination. To be eligible for contract award, vendor must be registered in the System for Award Management (SAM) and must also complete Representations and Certifications at http://www.sam.gov, per FAR 52.204-7. All payments by the Government under any associated purchase order shall be made by electronic funds transfer (EFT), per FAR 52.232-33. Proposals will be accepted to the close of business at 5:00 pm MST on September 8, 2017. Late proposals will not be accepted unless post marked prior to the time and date of the due date. ONLY ONE AWARD WILL BE MADE AS A RESULT OF THE SOLICITATION.
