Description: (i) This is a combined synopsis/solicitation for (1) Wall Mounted ENT Microscope System, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is VA244-17-Q-1651 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS code 339112 has a small business size standard of 1000. ITEM DESCRIPTION ENT Microscope Base Unit A single base ENT microscope is required to provide clinicians the ability to conduct ENT procedures in an outpatient medical clinic. The specific procedure for which this ENT microscope will be used will be cerumen removal from the ear and the vendor s provided equipment must be designed and manufactured for the intent of use in this procedure. Wall Mount with Articulating Arm Articulating arm to mount the ENT microscope to the wall of the VA Butler exam room in which this ENT microscope system will be installed Dustcover A dust cover to cover and protect the housing of the ENT microscope base and components detailed below must be provided. Sterilizable set of rubber caps A set of sterilizable rubber or soft-plastic caps must be included for the ENT microscope s various adjustment knobs and/or openings to minimize bacterial contamination and enhance ability to clean and sterilize the ENT microscope. Magnification Changer with 5 Steps A manual apochromatic magnification changer which provides 5 steps of manual magnification change must be integrated onto the ENT microscope base to allow for clinician s magnification changes. Halogen Lamp Plus Housing for Wall Mount Arm The wall mount arm, near its terminal, distal end to which the ENT microscope base is attached should provide a 100 watt halogen lamp with associated housing so that the area viewed through the ENT microscope can be illuminated to enhance visibility for the clinician operating the ENT microscope. 180 ° Tiltable Binocular Tube A 180 ° tiltable binocular tube is required as a viewing device for the clinician. This wide tilt range is necessary as it minimizes clinician operator fatigue. 12.5x wide-field eyepiece The ENT microscope must provide two 12.5x magnification eye pieces (i.e., one eye piece for the left eye and one eye piece for the right eye to be used simultaneously on the ENT microscope) that provide a wide-field view. Objective Lens F=250mm with Focusing The focus of the objective lens for the ENT microscope must provide for a f = 250 mm Handgrip set One T-shaped hand grip is needed to be mounted to the ENT microscope base so that the clinician operating the device may use either their left or right hands (or both hands simultaneously) to manipulate the positioning of the ENT microscope and articulating, wall mounted arm. Maneuverable Coupling for Wall Mount Arm To attach the ENT microscope base to the wall mounted arm, a maneuverable coupling is required so that the clinician can manipulate the spatial orientation of the ENT microscope base at the terminal articulation of the distal end of the wall mounted arm.
