Description: SCA WD 15-4555 (Rev.-3) 45 SFS Badge-Visitor Request Form Cathodic Protection Service Solicitation (SF 1449) Performance Work Statement CLIN Structure Breakout Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number FA252117R0027 shall be used to reference any written quote provided under this request for quote. The solicitation document (see attachment 1) and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95 effective 19 Jan 2017; DFRAS DPN 20161222 effective 22 Dec 2016; and AFFARS AFAC 2017-0127 effective 27 Jan 2017. THIS PROCUREMENT IS BEING ISSUED AS SMALL BUSINESS 100% SET-ASIDE. The North American Industry Classification System (NAICS) code for this project is 541990 with a size standard of $15M. REQUIREMENT: 45 CES/CEOS, Patrick Air Force Base (PAFB), FL requires a contract for the following services: The survey, inspections, and repairs to the cathodic protection systems on Patrick Air Force Base (PAFB). The contractor shall provide all engineering services, supervision, labor, tools, supplies, and equipment necessary to ensure that cathodic protection services being performed in a manner that will comply with government and commercial standards for cathodic protection services. Contractor shall conduct initial close interval, anode bed, and annual corrosion surveys of installed impressed current systems, perform initial and annual water tank calibrations, tank to water potential tests, and conduct annual leak investigations as required. (Attachment 1) All work will be accomplished in accordance with the Performance Work Statement (PWS) and all OSHA, A.W.W.A. Standards, ANSI, local, state, and federal regulations. The wages for this quotation shall be based on the attached U.S. Department of Labor, Employment Standards Administration Wage and Hour Division under Service Contract Act (SCA) Wage Determination WD 15-4555 (Rev 3) dated 01 Aug 2017.
