INVOICES: Invoices shall be submitted in arrears in accordance with: a. 852.232-72 Electronic Submission of Payment Requests (Nov 2012) b. SCHEDULE OF SUPPLIES/SERVICES AND PRICES/COSTS (SF 1449, Continuation of Blocks 19 24) Contractor shall provide the following labor, tools, and material to manufacture and install a counter top at the Jesse Brown VAMC, Chicago, IL. All materials shall be delivered FOB Destination. Installation shall be completed 60 days after receipt of award. Offeror shall provide the following: Item No. Description QTY UNIT UNIT PRICE EST TOTAL PRICE 1 Labor, tools, and materials to manufacture and install a counter top per the below specifications 1 job TOTAL Salient Characteristics: Metal fabrication with 1 chemically-resistant black countertop to support a laboratory workstation Capable of supporting a laboratory instrument weighing approximately 400 lbs. Rectangular-shaped, free-standing configuration with no less than 33 deep countertops Dimensions to fit an area no larger than 160 wide x 96 long. Cabinetry to be at bench height 12 wide, contained chase to accommodate the pull of electrical wires for sockets Double gang, double face electrical pedestal fixture Double gang, double face data pedestal fixture Expandable knee space panel 1 drawer in lower base unit 1 door lower base unit cabinet Pre-drilled service hole in countertop for electrical/data lines Debris removal including all cartons, crates and packaging materials accumulated as a result of the installation. Clean-up of job-site upon completion of installation
