This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for quote (RFQ) and it does not obligate the Government to award a contract. Request for a solicitation will not receive a response. The purpose of this sources sought announcement is for market research to make the appropriate acquisition decisions and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 621492 Kidney Dialysis Centers. The Department of Veterans Affairs, Network Contracting Office (NCO) 22 is seeking sources to provide the following professional services at the VA Long Beach Healthcare System (VALBHS) Medical Services Department. Routine Bedside Dialysis - CPT Code# 90935 Rate Per Procedure Urgent Bedside Dialysis CPT Code# 90935 Rate Per Procedure Emergency Bedside Dialysis CPT Code# 90935 Rate Per Procedure Plasma Exchange CPT Code# 36514 Rate Per Procedure All interested offerors who are capable of providing these products shall provide a written response to this notice and include the following: Company name, Address, Point of contact, Phone, fax, and email, DUNS number, Cage Code, Tax ID Number, Type of small business, e.g. Services Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), HUBZone, Women Owned Small Business (WOSB), Small Disadvantaged Business, or Small Business [NOTE: If you are a SDVOSB or VOSB, you must submit your certification from the Center for Verification and Evaluation (CVE) (https://www.vip.vetbiz.gov/)], Provide a copy of your company s current Representations and Certifications from www.sam.gov, Provide evidence your firm can supply the equipment and services for this Request for Information (RFI). Firms shall provide the following information on current and/or previous contracts in which you have provided this equipment. Furthermore, the Government may reach out to your listed point of contact to verify information provided in your response: Contract number Facility s Name and Address Point of contact, to include name, title, phone number, and email of the facility in which you have contracted with. Must complete and sign the following Word Document attached to this notice: S02 RN QUAL LIST HEMODIALYSIS If possible, provide an estimate rate for these services on a per procedure basis. All responses must be received no later than August 11, 2017, 12:00 PM Pacific Daylight Time (PDT).
