- The NAVSUP Fleet Logistics Center (FLC), Norfolk, Contracting Department, Philadelphia Office intends to acquire on a sole source basis with UpToDate, Inc., virtual evidence based clinical support services for the Bureau of Medicine and Surgery Enterprise. Navy health care professionals require access to an in depth, accurate, complete, unbiased, evidence-based treatment plans and peer reviewed clinical information to be used at the point of care (chairside). This is information is preferred for clinicians that have limited or little experience in specific areas of medicine. The treatment plans shall be distinct and provide guidelines for how a resolution to a patient medical condition should occur. This electronic clinical reference topical review database subscription is required to support our junior physicians, nurse practitioners, physician assistants and independent duty corpsmen (IDC) with the ability to input symptoms that a patient presents to them leading to a list of possible diseases or disorders that they can then narrow down to the actual disease, disorder or condition that they have leading to treatment of the patient. This is especially needed as many of our remote located clinics do not have easy access to consultation with specialist providers. These providers are often the sole providers where they work. These are also the providers that work on Navy vessels, both submarines and ships. Patient education information in the form of handouts that can be printed out is integral to this product. The service shall also provide a resource of vetted articles that are suitable to provide to patients. This topical review database is required for physicians and medics as a first-line comprehensive and primary source of information. Services shall include: - Provide full-text, comprehensive topical medical reviews, referenced with current, authoritative medical literature hyperlinked to MEDLINE abstracts. - Provide content that is based on current literature and reviewed and have major content updates at least 3 times per year. - Provide content that is written at the medical provider level and be written by expert physicians in the specialty and peer reviewed by other experts in the specialty; credentials and affiliations of the authors and reviews shall be included in the content. - Provide content that covers (at a minimum): o Adult and Pediatric Emergency Medicine o Adult Primary Care and Internal Medicine o Allergy o Immunology o Cardiovascular Medicine o Dermatology o Drug Information o Endocrinology o Family Medicine o Gastroenterology o Hematology o Infectious Diseases o Nephrology o Hypertension o Neurology o Obstetrics, Gynecology and Women ™s Health o Oncology o Pediatrics o Psychiatry o Pulmonary Critical Care and Sleep Medicine o Rheumatology o Surgery - Provide content that also covers pathophysiology, diagnosis, prognosis, etiology, laboratory tests, and recommended treatment of medical conditions with grading of evidence if appropriate. - Provide content that is synthesized into clear and concise answers. - Provide content that includes both pediatric and adult topics and the ability to sort content by age group. - Provide interactive medical specialty calculators, i.e. cardiac output, peak expiratory flow, etc. - Provide a fully integrated drug database including drug to drug interaction information. - Provide prescribing, dosing, and adverse effects of drugs listed for treatment reviews. - Provide information about laboratory tests including normative values, contraindications and risks. - Include a fully tested, functional and of commercial quality DVD (s) containing all the same topic reviews as the online version in their entirety. Due to the lack of internet connectivity on ships and deployed, remote locations, this requirement is of utmost importance. - Have search tools enabling users to find/retrieve all medical topic reviews on search topics in one interface including drop-down menus of possible matches after typing the first few characters. - Map from common synonyms, abbreviations, and acronyms. - Ensure users have the capability to print, email, and download full-text topic reviews. - Have a search interface that is customer-friendly and effective at retrieving relevant content quickly and easily. - Enable authorized users to retrieve full-text topic reviews that can stand alone. - Provide date of record/review update. - Provide a stand-alone complete database that does not require a subscription to other products to retrieve a complete medical topic review. - Provide alerting features to notify users of new content since the last update. Alert content shall focus on information that is patient-oriented and with the potential to change current practice of health care providers. - Provide a simple grading system for evidence. - Ensure content is searchable with words or phrases in titles and content and by subject. - Provide cross links to other relevant information in the database. - Provide references that link to PubMed and to subscribed content through an Open URL Link Resolver. - Provide patient education handouts within the database. - Provide continuing medical education credits certified by the Accreditation Council for Medical Education. Physicians shall receive AMA Category 1 CME for each search performed. Users shall be able to track CME activity after establishing an account. - Be compliant with śMyAthens ť proxy solution. - Be capable of multiple, simultaneous access via Internet. - Be comparable with Army and AF clinical reference database products. Due to the increasing movement toward joint military activities, as well as recognizing the impact of realignments and consolidations resulting from the BRAC, has prompted further consideration of seeking a tri-service solution for knowledge-based electronic medical resources. Particularly given that medical personnel from different services will be working side-by-side, ensuring equal access to a clinical reference database on which to assist in making clinical decisions, will help promote consistently high quality care and patient safety. - Provide telephone and email technical support 365/24/7. - Provide a monthly usage report to the DHA PM displaying the number of users, IP identification and the number of reviews accessed. FLC ™s intent is to process this procurement on a sole source basis to UpToDate, Inc. based out of Waltham, Massachusetts. FLC ™s intent is to issue a firm fixed price contract. The Period of Performance is from 01 September 2017 through 31 August 2018, plus four one-year option periods ending 31 August 2021, if all options are exercised. The proposed contract action is for services for which the government intends to solicit and negotiate with only one source under authority of FAR 6.302. Interested offerors may identify their interest and capability to respond to the requirement or submit a capability statement. This notice of intent is not a request for competitive proposals. However, all capability statements received by 1600 on 31 July 2017 will be considered by the government. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
