1 STATEMENT OF WORK Nurses Station Department of Veterans Affairs Veterans Health Care System of the Ozarks Leroy Pond Domiciliary Fayetteville, Arkansas June 15, 2017 INTRODUCTION These specifications describe the requirement for procurement, delivery, and installation of a Nurses Station for the Leroy Pond Domiciliary, located at the Veterans Health Care System of the Ozarks, Fayetteville, Arkansas. The Nurses Station will consist of various items and configured using the products listed below in the salient characteristics. SCOPE OF WORK OVERVIEW This project shall consist primarily of field verification, receipt, delivery and installation of the products and hardware listed below. Items specified are based on the characteristics of cabinets, walls, and countertops currently being utilized by the contractor responsible for construction of the building. SPECIFICATIONS The suggested manufacturer is WKI Casework materials OR EQUIVALENT. Construction to include an L-Shaped Nurses Station complete with panel walls, two (2) straight raised counter tops reception counter, full table legs, and end caps, wall mounted laminate cabinets, and a wall mounted laminate countertop. See Attachment A and Attachment B for additional design/layout information. LOCATIONS Items shall be delivered and installed by the contractor at the Leroy Pond Domiciliary located at 1100 N College Ave, Fayetteville, AR 72703 PRODUCTS / SALIENT CHARACTERISTCS All products must meet or be equivalent to all salient characteristics defined in this section. This is inclusive of all products, works surfaces and finishes. ENVIRONMENTAL INITIATIVES It is important to the VA that the products provided are manufactured with sustainability and environmental attributes in mind. Aluminum and extruded products for interior and exterior shall feature 75% recycled content. All composite board or hard board products shall be EPP Downstream certified for low formaldehyde content and sustainable practices. All table systems must be manufactured in the United States of America. DELIVERY AND STORAGE COR shall be contacted no less than 2 weeks prior to delivery to schedule installation at the VHSO facility in Fayetteville, AR. Contractor shall conduct delivery and installation between September 18th-29th, 2017, Monday through Friday, between 8:00am and 4:30pm, ONLY. Attempts to deliver furniture items prior to September 18th will be refused upon arrival. The contractor will be responsible for coordinating delivery of items between all involved parties, including the facility COR, the installation contractor and the manufacturer. Delivery and unloading shall be at the Leroy Pond Domiciliary at 1100 N College Ave, Fayetteville, AR Deliver materials to job in manufacturer's original sealed containers with brand name marked thereon. Package to prevent damage or deterioration during shipment, handling, storage and installation. Maintain protective covering in place and in good repair until removal is necessary. Deliver products only when the site and mounting services are ready for installation work to proceed. Store products in dry condition inside enclosed facilities. Product must ship directly to the Fayetteville, AR Leroy Pond Domiciliary. Vendor is prohibited from receiving or storing product on behalf of VA. Vendor installation team shall be present at time of delivery to offload and stage all furniture and equipment in the VA designated location. The facility will not provide staff to assist with this task. CONTRACTORS RESPONSIBILITES DESIGN SERVICES, DRAWING & PROCUREMENT REQUIREMENTS The Contractor shall provide Design Services, Procurement and Project Management. The Contractors Customer Service Representative (CSR) will coordinate layouts and installation as directed by Fayetteville Veterans Health Care System of the Ozarks (VHSO) COR and adhere to the following requirements. As Built Digital Renderings to include measurements and dimensions of specified products shall be submitted prior to award to ensure items meet technical requirements. For each type of exposed finish required, provide a sample, not less than three (3) inches square, of same thickness and material indicated for the work. Sample(s) shall be submitted within 10 business days of award. If finishes involve normal color and texture variations, include sample sets showing the full range of variations expected. SITE VISITS The Vendor shall coordinate the following site visits with the Fayetteville Veterans Health Care System of the Ozarks (VHSO) COR and the vendor s appointed CSR. A post-award walkthrough 7 days prior to order placement to field verify all measurements and existing conditions. The contractor shall revise the drawings and quotes to reflect changes due to unforeseen construction conditions or discontinued product. A post-installation walkthrough upon project completion. The CSR must be present to asses, address, and document any punch-list items. INSTALLATION Installation of specified products shall be scheduled in one phase. The contractor will be required to install all products solicited under this RFP. The installation team must always have a professional appearance and demeanor. Laborers should display either a company logo or company name badge on his or her attire. All labor is to be furnished by the contractor and must be appropriately estimated. All work is to be completed in the highest level of craftsmanship and in a safe manner and in the allotted time frame. The contractor is to maintain a clean job site during work hours and properly store materials and supplies at the end of each work day. All dust and debris must be cleaned by the contractor- this includes vacuuming and damp mopping when necessary. Cleaning equipment and supplies will be the responsibility of the contractor and will not be provided by the Fayetteville Veterans Health Care System of the Ozarks (VHSO). All packaging and waste is to be recycled and/or legally disposed of by the Contractor. FINAL INSPECTION & ACCEPTANCE The CSR will be required to do a post-installation inspection upon project completion and project sign-off. The CSR will photograph and document all damaged, deficient, defective or mis-ordered/shipped products at the job site and submit them to the manufacturer for repair or replacement. Upon punch-list completion, CSR is to submit to the COR a remediation plan to correct deficiencies within two (2) working days Contractor will correct deficiencies and punch list items within ten (10) business days after identification, unless deficiency requires fabrication of replacement parts/pieces in which the time allowance will be increased to thirty (30) calendar days for non-readily available or stock items. CLOSE OUT DOCUMENTATION One complete set of documentation, as listed below, shall be provided to the VHSO COR at the completion of installation. Complete Binder to include product brochures, specified fabric memos samples and technical recommendations for maintenance and service of the materials used. PROTECTION OF PROPERTY Contractor shall protect all items from damage. The Contractor shall take precaution against damage to the building(s), grounds and furnishings. The Contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor. SECURITY REQUIREMENTS The Certification and Accreditation requirements do not apply and a Security Accreditation Package is not required.
