- Sow Replacement of Carpeting Project July 25, 2017 Statement of Work Project Name & Location Replacement of existing carpet tiles in designated areas of the U.S. Probation Office – EDNY located at the Alfonse M. D’Amato U.S. Courthouse & Federal Building, 100 Federal Plaza, Central Islip, NY 11722. The designated areas of carpet replacement will be identified and disclosed during the walkthrough. Extensions will be granted for unforeseen conditions, and other factors outside of the contractor’s control. Project Scope The United States Probation Office EDNY has the requirement to replace carpet tile in designated offices/ areas within their office space located at the Alfonse M. D’Amato U.S. Court and Federal Building. It is anticipated that the existing carpet tile be replaced with 24” x 24” carpet tile. Re-carpeting will be limited to the spaces designated by Agency Personnel and will be provided during walkthrough. The contractor shall coordinate the phasing of this project with the U. S Probation and GSA to ensure minimal disturbance to probation operations. Carpeting The contractor shall provide all of the labor, tools, material handling, logistics, moving and reinstallation of office furniture and installation materials, i.e. carpet, adhesive, etc. required to demolish and remove the existing carpet and replace it with carpet tile chosen by the customer agency with consultation provided by GSA. 2. The contractor will be responsible for verifying all field measurements and providing Carpet quantities necessary to successfully complete the project and also for providing enough overage for attic stock. 3. Carpet quantities should be listed and broken down in the attached Contract Price Form. 4. Proposals will be evaluated on the attached Contract Price Form and should be included on your price breakdown. 5. The contractor is responsible for researching, understanding, and complying with the manufacturer’s installation directions which can be found at manufacturer’s website. Any deviation from the manufacturer’s installation directions must be approved by the GSA Project Manager. 6. The contractor shall provide all labor, materials and equipment required to prepare all sub surfaces in accordance with carpet manufacturer installation directions to ensure a proper installation. 7. The contractor shall supply a dumpster and dispose of all debris and material created during the demolition of the existing carpet tile. The contractor shall recycle all recyclable material and provide documentation to GSA as part of the close out documents. 8. The contractor shall take all precautions to eliminate the possibility of a trip hazard in any section of carpet that is partially installed adjacent to existing areas of carpet. 9. The contractor shall supply all transition strips as necessary. Prices Basis of Pricing (1) Contract Prices. All Contract prices set forth in this Section include all costs necessary to complete the work for which the price is established (e.g., Base Contract, Unit Price,) in accordance with the Contract Documents, including, but not limited to, the cost of work performed by subcontractors and consultants, indirect costs, fees, expenses, taxes, and profit. (2) Knowledge of Conditions Affecting Price. FAR 52.236-3, Site Investigations and Conditions Affecting the Work, is incorporated by reference in this Contract. The Contractor shall be presumed to have established all prices with knowledge of general and local conditions that may affect the cost of Contract performance at the site where the Work is to be performed, to the extent that such information is reasonably obtainable. (3) Unit Prices and Allowances. If any portion of the Work is to be performed on a unit price basis, the Unit Price shall include all costs of coordinating and incorporating the unit-priced portion of the Work into the Base Contract Work. The Contractor shall only be obligated to perform unit-priced work to the extent that an Allowance has been established. The Contractor shall be obligated to perform such work in excess of a unit quantity for which an Allowance is established only if directed by the Contracting Officer in writing. The Contractor shall be bound to the unit price or prices set forth herein in all equitable adjustments for changes including unit priced work, and no markups shall be applied to such unit prices. Contract Price Form The below line items should be included on your price breakdown. Your proposal will be evaluated based on the below information. CLIN ITEM PRICE 1 Cost for providing all labor, oversight and materials to complete work as per SOW. $ (a) Cost of Carpet/adhesive (Please verify & Confirm field measurements and indicate Square Yards of carpet ). $ (b) New Carpet Tile: (State Color/Make/Model) $ (c) Removal of existing Carpet Tile $ (d) Installation of New carpet tile $ (e) Furniture Moving $ (f) Material Handling $ (g) Carpet Adhesive $ (h) Contractor on site Supervision $ (i) Indicate (and attach a description of) other costs necessary for successful project completion (other costs not listed in this contract price form i.e. material & supplies). These items should be broken down by material, equipment and labor. Attach supplemental sheet if necessary. $ (j) Indicate Overhead and Profit costs $ Total Contract Price $ Proposal pricing instructions: When submitting a detailed price proposal provide a cost breakdown for each task and include other costs necessary for successful project completion. Be sure that costs are broken down for Materials, Equipment and labor. Davis Bacon Wage rates apply. Describe any special conditions related to the project. Please indicate carpet square yards needed to successfully complete the project and provide line item price on line 1(a). Other Special Instructions - Work Schedule All work involving carpet demolition and installation shall be performed after normal working hours. If the tenant agency agrees, some work may be performed during normal working hours for certain areas of the office. The authorization of work during normal working hours will be made on a case by case basis. All furniture removal will be performed during normal and after normal working hours as needed in coordination with the tenant agency contact. Site Management – Safety The contractor shall identify an on-site project manager / supervisory who shall be on-site whenever contractor personnel are working on this project. This person will be the primary point of contact between the contractor personnel and the GSA project manager. The project site shall be kept in a neat, orderly and safe condition at all times. The contractor shall provide enough containers for collection of construction debris and construction material to be recycled. No construction material may be left in the work area while work is not ongoing. For example, at the end of the work shift, all material, debris, tools, etc, must be removed from the work area and staged in the designated area between shifts. This provision applies specifically to the work in the secure hallways. The work site will continue to be in use for the duration of the project. The contractor shall provide a barricade or otherwise block off the immediate work area to prevent unauthorized entry. Tools, Equipment and Material - Storage The contractor shall provide all tools, equipment and materials required to perform the work associated with the entire project. The contractor shall operate in a safe manner and adhere to all OSHA and GSA standards. On-site storage of equipment and materials shall be provided by GSA at the discretion of the GSA Building Management. Deliveries The contractor shall provide notice to GSA Building Management of all deliveries 24 hours in advance. The contractor shall be responsible to have personnel on-site to receive and provide material handling of all deliveries. If deliveries are unannounced and/or contractor personnel are not onsite to receive the delivery, the delivery will be refused and turned away. The Government shall not be responsible for any additional costs associated with rescheduling the delivery. Loading Dock Information – the loading dock is below grade with ramp access. The loading dock is 12 feet 5 inches in height. Freight Elevator – (1) Freight elevator – Weight Capacity is 5000 lbs., Cab size – 9’ long, by 5’5” wide by 9’ high. Demolition Debris and Material Removal The contractor shall provide dumpsters as necessary to handle all demolition debris or excess materials from the project. The location of these dumpsters will be coordinated with GSA at the start of the project. The contractor shall be responsible for the transportation of the waste and/or recyclable material to a landfill / transfer station as well as any disposal costs. All recycled materials shall be sent to recycling plants and manifest from the plant will be provided to GSA for documentation. The contractor shall not stockpile demolition debris within the building at any time. Progress Meetings Formal progress meetings are not anticipated during the duration of this project. The GSA project manager shall make daily inspections of the work progress. Cleaning Before scheduling the final inspection, the contractor shall (1) Remove all tools, equipment, surplus material, and rubbish; (2) Restore or refinish, surfaces that are damaged due to work on this contract; (3) Remove grease, dirt, stains, foreign materials and labels from finishes surfaces; (4) Thoroughly clean building interiors within the designated work areas; (5) Dispose of all construction debris from the site; and (6) Ensure that at the time of final inspection, project shall be thoroughly cleaned and ready for use. Environmental Issues The contractor shall submit all Material Safety Data Sheets for all adhesives and solvents that will be used for this project. Security All contractor and sub-contractor personnel working on-site on this project must submit to the HSPD-12 security background check. Please note contractor personnel must travel to 26 Federal Plaza to be fingerprinted as part of the background check. Administrative Matters Substantial Completion The Work shall be deemed "Substantially Complete" if and only if the Contractor has completed the Work and related Contract obligations in accordance with the Contract Documents, such that the Government may enjoy the intended access, occupancy, possession, and use of the entire Work without impairment due to incomplete or deficient work, and without interference from the Contractor's completion of remaining work or correction of deficiencies in completed work. In no event shall the Work be deemed Substantially Complete if all fire and life safety systems are not tested and accepted by the Authority Having Jurisdiction, where such acceptance is required under the Contract. Schedule of Values The Contractor shall prepare and submit for approval a detailed cost breakdown of the Contract price, to be referred to as the Schedule of Values, assigning values to each component of the Work. Values must include all direct and indirect costs, although a separate value for bond costs may be established. The Schedule of Values must contain sufficient detail to enable the Contracting Officer to evaluate applications for payment. If this Contract requires that the Project Schedule be cost loaded, the Schedule of Values will be derived from the Project Schedule. Payments Please comply with the provisions of your contract for Payments under FAR 52.232-5 “ Payments under fixed-price construction contracts ". (4) Prompt Payment. In accordance with FAR clause 52.232-27, the period for payments is as follows: (a) Progress Payments: 14 days (b) Subsequent Subcontractor Payments: 7 days (5) Payment Information. The General Services Administration (GSA) makes information on contract payments available electronically at http://www.finance.gsa.gov. The Contractor may register at the site and review its record of payments. This site provides information only on payments made by GSA, not by other agencies. Bid Guarantee Please comply with the provisions of your contract under FAR 52.228-1. FAR 52.228-13 Alternative Payment Protections (July 2000) (a) The Contractor shall submit one of the following payment protections: (i) A payment bond. (ii) An irrevocable letter of credit (ILC) (b) The amount of the payment protection shall be 100 percent of the contract price. (c) The submission of the payment protection is required within _7_ days of contract award. (d) The payment protection shall provide protection for the full contract performance period plus a one-year period. (e) Except for escrow agreements and payment bonds, which provide their own protection procedures, the Contracting Officer is authorized to access funds under the payment protection when it has been alleged in writing by a supplier of labor or material that a nonpayment has occurred, and to withhold such funds pending resolution by administrative or judicial proceedings or mutual agreement of the parties. (f) When the tripartite escrow agreement is used, the contractor shall utilize only suppliers of labor and material that signed the escrow agreement.
