2 Statement of Work CB EQ049 Eye Clinic Equipment Eastern Colorado Health Care System Denver, CO 03/01/17 PURPOSE 1.1 The overall purpose is to deliver CB EQ049 - Eye Clinic Equipment at the Eastern Colorado Health Care System, 1700 N. Wheeling Street, Aurora, CO 80045. SCOPE The Contractor shall deliver all items listed in section 2.5. All items must meet all salient characteristics defined in section 2.5. 2.3. The Contractor shall furnish all personnel, supplies, and equipment required for delivery. 2.5 SALIENT CHARACTERISTICS 2.5.1. Ophthalmometer/Keratometer (JSN M5545) Quantity: 2 Basis of Design: Marco/Nidek Model ARK 1A or equal to Specifications/Salient Characteristics Shall be a multimodality device that includes autorefraction and keratometry and features accommodating testing and retro-illumination. Shall detect early presbyopia and latent hyperopia and features super luminescent diode (SLD) that accurately measure patients with cataracts, corneal opacities, IOLs, and post LASIK. Shall utilize a multiple pupil zone imaging method that combines rotary prism technology and auto alignment dye tracking system on the X-Y-Z axes. Shall provide printout of measured values. Shall have a summary display. Shall use double ring and SLD technology. Shall have a confidence index indicator. Table, Motorized, Optometry (JSN M5545.A) Quantity: 2 Basis of Design: Marco/Nidek Model ARK-530A Table or equal to Specifications/Salient Characteristics Shall be an electrical H-base table that can support the Ophthalmometer/Keratometer in Section 2.5.1 above. Approximate Dimensions: Adjustable Height Minimum/Maximum: 26 / 36 Approximate dimensions: 24.4 L x 18.0 W Power Requirements 120V, 60 Hz Accessories/Configuration Options: Four (4) lockable casters 2.5.3. Pachymeter Ultrasound Corneal (JSN M6040) Quantity: 3 Basis of Design: DGH Technology Model Pachmate 2 Handheld Pachymeter or equal to Specifications/Salient Characteristics Portable device designed to measure corneal thickness accurately and quickly. The item shall be capable of obtaining and storing up to 25 measurements within a second. The item shall have an LCD display to indicate the current measurement, average, and standard deviation of all measurements taken. The item shall provide audible feedback to indicate when each measurement is complete. The item shall measure in the range of 200 to 1100 microns, with an accuracy of +/- 5 microns and a resolution of 1 micron. The item shall have a data transfer interface to a PC. The item shall be battery operated. Shall have bluetooth capability. Technical Specifications: Measurement Range: Standard Range 200 microns to 1100 microns, Flap Option Range 95 microns to 11 microns Accuracy: +-5 microns Resolution: 1 micron Connect software system requirements 32 bit or 64 bit, GHz processor, 2 GB RAM, Hard Drive 1GB minimum 100 GB recommended, USB 2.0 port, 1024 x 768 resolution display, mouse, keyboard Connect software compatibility Windows XP, Vista, 7 or 8/8.1 Approximate Dimensions: 7.1 L x 1.6 W x 0.9 D Power Requirements Two (2) AA Batteries Accessories/Configuration Options: AA Batteries 2.5.4. Thermal Paper for Ophthalmometer/Keratometer Quantity: One 12 pk Roll Shall be designed for use with ophthalmometer/keratometer in item 2.5.1. above 2.5.5. Operator Training Contractor shall provide all training materials at time of delivery. Contractor shall provide on-site training on JSN M5545 (Macro/Nidek Model ARK1A or equal) for 8 individuals. DELIVERY DELIVERY Delivery Timeframe: 60 Days ARO Contractor is responsible for relaying all delivery requirements to any third party delivery. Acquisition of the above items is part of the initial outfitting and activation of the new VA medical center being constructed in Aurora, CO. Since construction is still occurring, delivery dates are approximate and will be coordinated with the successful contractor after award (see 2.6.1.8 Coordination). The Contractor shall contact the Onsite POC to schedule a pre-delivery meeting to be conducted approximately 30 days prior to the initial award delivery date for verification of delivery dates. The Contractor may be required to adjust the delivery date from the date specified in the contract award document. A government-requested delivery delay up to 90 days after the delivery date specified in the contract award shall be at no additional cost to the government. Delivery Location: Contractor shall deliver all equipment to the Eastern Colorado Health Care System, Research Dock, 1700 N. Wheeling Street, Aurora, CO 80045. Items on delivery vehicles not equipped with a Lift Gate will not be accepted. The delivery will have to be rescheduled and any delivery charges will be the responsibility of the Contractor. Delivery Markings: Contractor shall deliver items in manufacturer's original sealed containers with manufactures s name marked thereon. Deliveries shall be marked with the PO and contract number. Delivery will be coordinated through the Onsite POC. Delivery personnel must comply with all posted site safety requirements, to include the wearing of PPE. Minimum requirements are hard hat, over the ankle boots, reflective safety vest, eye protection and gloves. If the requirements in sections 2.7.1 and 2.7.2 are not adhered to the delivery will be rejected and be rescheduled at Contractors expense. INSPECTION AND ACCEPTANCE 3.1. The Contractor shall conduct a joint inspection with the Onsite POC upon completion of delivery. 3.2. In the event deficiencies are identified, the Contractor shall provide date when the identified deficiencies will be addressed if not addressed on the date of delivery. 3.3. The Contractor shall conduct a joint inspection with the Onsite POC after addressing all deficiencies. 3.4. All deficiencies identified during joint inspections shall be fixed by the Contractor prior to government acceptance of the item. 3.5. Disputes shall be resolved by the Contracting Officer. Operation and Maintenance Manuals Binders - Quantity (2) each for items identified in section 2.5. 4.2. Digital Copies - Quantity (1) each for items identified in section 2.5. 4.3. Deliver compilation of all manufacturer recommended maintenance schedule and operation materials packaged in binder(s) to Onsite POC upon completion of delivery. 6. PROTECTION OF PROPERTY 6.1. Contractor shall protect all items from damage during delivery. The Contractor shall take precaution against damage to the building(s), grounds, and furnishings. The Contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor. 6.2. The Contractor shall perform an inspection of the building(s) and grounds with the Onsite POC prior to commencing work. The Contractor shall be responsible for repairing or replacing any items, components, building(s), or grounds damaged due to negligence and/or actions taken by the Contractor or its employees. Concurrence from the VA Facilities Management POC and Onsite POC is required before the Contractor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade, and color to match adjacent existing work. 6.3. The Contractor shall be responsible for securing the items identified in section 2.5 and its own work tools and equipment during delivery. 7. SECURITY REQUIREMENTS 7.1. Security background checks are not required. 8. WARRANTY 8.1. The contractor shall provide all manufacturer(s) warranties on all parts and labor at time of delivery. 8.2. The warranties shall include all travel and shipping costs associated with any warranty repair.
