- CANCELLATION NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is W912MM-17-T-0046 and is issued as an invitation for bids (IFB), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated North American Industrial Classification System (NAICS) code for this procurement is 2005-95 with a small business size standard of 500.00 employees. This requirement is a [ Small Business ] set-aside and only qualified offerors may submit bids. The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2017-07-26 14:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com. FOB Destination shall be in the Statement of Work. The National Guard - South Dakota requires the following items, Brand Name or Equal, to the following: LI 001: The South Dakota National Guard Medical Command is requesting 16 Hearing Booths and Supporting Audio meter equipment, and all supporting equipment and computer software that corresponds to the audiometers. Item descriptions and manufacturer reference information provided for the requested Hearing Booths and Audiometer equipment is provided to identify a desired standard of performance and quality. A standard level of quality would be comparable to the following description. 1.Acoustic Systems RE-125 This reference shall not be construed as limiting competition. Items quoted must be equal to or better than what ™s identified. Products quoted are required to meet minimum specifications. Hearing Booth Salient Characteristics: Acoustic floor with casters Prewired jack panel with 1 1/2" diameter pass-thru. Prewired jack panel contains 4 each 1/4" phono stereo plugs. Acoustically engineered integrated ventilation system Acoustical safety glass window, 18" (wide) by 24" (high) Carpeting/Sound deading foam lighting Electrical service via a 12', 12 volt power supply Preassembled Acoustic roof and floor Acoustic Wall Panels Qty 8 = Right Hinged doors Qty 8 = Left Hinged doors Acoustical Performance: Noise Reduction, 16, EA; LI 002: AUDIOMETER Item descriptions and manufacturer reference information provided for the requested Hearing Booths and Audiometer equipment is provided to identify a desired standard of performance and quality. A standard level of quality would be comparable to the following description. 1.BENSON 200 Mini System Audiometer This reference shall not be construed as limiting competition. Items quoted must be equal to or better than what ™s identified. Products quoted are required to meet minimum specifications. a.Needs to have the compatible software for the DOEHRs (Defense Occupational and Environment Health Readiness System) program. b.Needs to be OSHA compliance., 2, EA; LI 003: EXPANSION AUDIOMETER Item descriptions and manufacturer reference information provided for the requested Hearing Booths and Audiometer equipment is provided to identify a desired standard of performance and quality. A standard level of quality would be comparable to the following description. 1.BENSON CCA 20 Mini Expansion Audiometer This reference shall not be construed as limiting competition. Items quoted must be equal to or better than what ™s identified. Products quoted are required to meet minimum specifications. a. This item does not require the same software as item 20 b. Needs to be OSHA compliance, 30, EA; LI 004: SIMULATOR Item descriptions and manufacturer reference information provided for the requested Hearing Booths and Audiometer equipment is provided to identify a desired standard of performance and quality. A standard level of quality would be comparable to the following description. 1.BENSON BAS 200 Simulator This reference shall not be construed as limiting competition. Items quoted must be equal to or better than what ™s identified. Products quoted are required to meet minimum specifications. a. Quick and accurate daily calibration check as required by OSHA 29 CFR 1910.95. b. Uses no batteries power needs to be supplied from the audiometer. c. Works with standard earphones and audio cups. f. Audiometers need to be DOHRE Software-compatible., 16, EA; LI 005: Delivery will be to two locations: 2823 West Main St, Rapid City SD 57702 801 West National Guard Drive, Sioux Falls SD 57104 Each Location will receive: 4 Right hinged door hearing booths 4 Left hinged door hearing booths 1 Audiometer 15 Expansion audiometers 8 Simulators, 1, EA; LI 006: Quote requirements Please include the following with your quote: 1. Failure to submit requested documents will cause you to be consider noncompliant **Vendor is required to invoice and be paid by Electronic Funds Transfer through Wide Area Work Flow, 1, EA; Solicitation and Buy Attachments ***Question Submission: Interested offerors must submit any questions concerning the solicitation at the earliest time possible to enable the Buyer to respond. Questions must be submitted by using the 'Submit a Question' feature at www.fedbid.com. Questions not received within a reasonable time prior to close of the solicitation may not be considered.*** For this solicitation, National Guard - South Dakota intends to conduct an online competitive reverse auction to be facilitated by the third-party reverse auction provider, FedBid, Inc. FedBid has developed an online, anonymous, browser based application to conduct the reverse auction. An Offeror may submit a series of pricing bids, which descend in price during the specified period of time for the aforementioned reverse auction. National Guard - South Dakota is taking this action in an effort to improve both vendor access and awareness of requests and the agency's ability to gather multiple, competed, real-time bids. All responsible Offerors that respond to this solicitation MUST submit the pricing portion of their bid using the online exchange located at www.FedBid.com. There is no cost to register, review procurement data or make a bid on www.FedBid.com.
