Description
This request for information/sources sought notice is for planning purposes ONLY. The results of this market research will assist in the re-evaluation of the acquisition strategy in accordance with recent VA Procurement Policy guidance. VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice and does not guarantee an RFQ will follow. Veterans Health Affairs, Network Contracting Office 4, is issuing this request for information/sources sought notice in order to identify small businesses, and specifically, Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Business (VOSB) sources that have the resources and capabilities to provide and install one (1) Brand Name or Equal HD03 standalone Hemodialysis Monitor with a rechargeable battery for portability as defined below for the Philadelphia, PA VA Medical Center Summary The request is for the procurement of the HD03 standalone Hemodialysis Monitor with a rechargeable battery for portability. The monitor has a touch screen. The monitor is equipped with a Data transfer Module (DTM) which stores patient information. According to Director of the Renal Department the monitor will be used only to detect fluid levels pre and post a patient s dialysis treatment. The monitor, which has the capability to connect to a PC for data transfer, will be used solely to access fistulas, and grafts, and catheters to measure flow and cardiac function. Project Scope The scope of work includes delivery of the HD03 Hemodialysis Monitor. Installation is included in price and will be coordinated with Philadelphia VA Medical Center Biomedical Department. Brand Name or Equal Transonic HD03 Features and Options: HD03-CO OR XTHD03-CO, Non-China Shipment, Power Cord-US, HD03 English Language, Flow QC 2 Boxes to be ordered, H4FX HD03 Sensor, DTM-CO Data Transfer Module for C/O, HFW1000 Saline Bag Warmer, HFW1000 Power Supply, Power Cord-US, HFW1000 English Language, ADT1018-40 Flow QC Clear Advantage 40/Box, HD03 Software and Installation, ADT1018 Notice, Addendum A+ HD0x CDE, HD0x installation instructions, transport case, HD03 Rolling Stand, Warranty Salient Characteristics: RECHARGABLE BATTERY PORTABLE Pole mountable Interactive touch screen Battery Power Indicator Lights Transport case Cardiac Output Data Modules Non-Cardiac Data Module Calibration Shipping: FOB Destination (included in pricing) Interested & Capable Responses NCO 4 is seeking responses from small businesses, particularly service disabled veteran owned (SDVOSB) and veteran owned (VOSB), who are interested in this procurement and consider themselves to have the resources and capabilities necessary to provide this product. This is in accordance with the Supreme Court decision known as The Rule of Two.
