Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NICHD-17-145 and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-95. The North American Industry Classification (NAICS) Code is 334510 and the business size standard is 1250 employees. However, this solicitation is not set aside for small business. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) intend to contract on a sole source basis with Electrical Geodesics, Inc (EGI) - 500 East 4th Avenue - Suite 200 - Eugene, OR 97401 to procure the following items: Item# 1401283 - 128 Channel Geodesic EEG System 400 w/iMac Includes: Net Amps 400, Hospital Grade Isolation Transformer, iMac, Articulated Arm, Net Station Software License and HASP, Net Amp Wall Mount Bracket, Net Support Kit, 3 Day On-Site Installation and Training Visit, Amp Warranty Item# 4732050 - HydroCel GSN 130: 128 Channel Pediatric Small 47-51 cm - Quantity (1) EA Item# 4732040 - HydroCel GSN 130: 128 Channel Pediatric Large 51-54 cm - Quantity (1) EA Item# 4732020 - HydroCel GSN 130: 128 Channel Adult Medium 56-58 cm - Quantity (1) EA Item# 4732030 - HydroCel GSN 130: 128 Channel Adult Small 54-56 cm - Quantity (1) EA Item# 4732010 - HydroCel GSN 130: 128 Channel Adult Large 58-61 cm - Quantity (1) EA Item# S-WAR-NET-1000-128 - Individual 128 Channel HydroCel Geodesic Sensor Nets - Quantity (10) EA, Extended Warranty: One year Item# 2552000 - Basic Support for: Small Lab, one-year support for labs with 1 to 4 EGI Systems - Quantity (5) EA Item# 2552115 - Engineering Services - Quantity (10) Hours The EGI NIRS/EEG cap is an all-in-one solution that can be combined with the labs' current Hitachi ETG-4000 NIRS system with a 128-channel geodesic EEG cap. He device has been exclusively calibrated with our current NIRS Hitachi system for simultaneous EEG-NIRS measurements. This combination is unique in the market, offering the most probing channels with a very reliable and user-friendly engineering. Currently, there are no other EEG/NIRS nets on the market using the geodesic cap design. This means that all other caps require putting gel in each electrode after application to the subject's head. For infants and children, this means a significantly longer application leading to subject fatigue and increased data loss. The EGI takes less than a minute apply giving them great advantage for quality data collection in infants. The Geodesic Sensor Net and the HydroCel Geodesic Sensor Nets are proprietary technology of EGI and available solely from EGI. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source under authority of the 41 U.S.C. 253(c) (1), FAR 6.302. The offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-1 Instructions to Offerors - Commercial Items; FAR Clause 52.212-2, Evaluation - Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation for Simplified Acquisitions applies to this acquisition. The offeror must include their Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Offers must also be accompanied by descriptive literature, delivery timeframe, warranties and/or other information that demonstrates that the offer meets all of the foregoing requirements. Quotations will be due fifteen calendar days from the publication date of this synopsis or by July 28, 2017 before 10:00am EST. via email or postal mail.
