- This is not a request for quotes or proposals. No formal solicitation document exists at this time. The Prosthetic and Orthotic Services at the James E Van Zandt VA Medical Center in Altoona, Pennsylvania has an ongoing program to furnish artificial limbs, orthotic Services and orthoses to those Veterans that require them and are eligible for assistance. There is an estimated 4,500 Orthotics and Orthotic devices as well as 30 prosthetic limbs each year. Some information from the Statement of Work contains the following: The Contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic Team for each item furnished. Contractor shall not add or substitute components or alter the beneficiary s limb prescription in any way without prior approval and written authority from the VA Amputee Clinic Team Prosthetic Representative or designee. The Contractor shall provide service including evaluation, manufacture, fitting and adjustment of Orthotic devices by its qualified Orthotist. An orthosis is a device fitted externally to an anatomical portion of the body to influence motion by assisting, resisting, blocking, or unloading part of the body weight. An orthosis may be used to correct deformity, compensate for weakness, or protect a body segment. It includes, but is not limited to, custom and non-custom devices, corsets, trusses, and belts. The overarching goal of the acquisition is to establish several Indefinite Delivery Indefinite Quantity (IDIQ) contracts to effectively and expeditiously fulfill their need over a five (5) year period. The James E Van Zandt VA Medical Center catchment Area is listed below and the vendor must be located in at least one of the locations within the following Pennsylvania Counties: Bedford, Blair, Cambria, Cameron, Centre, Clearfield, Clinton, Elk, Huntingdon, Indiana, Jefferson, Juniata, Mifflin and Somerset. This sources sought notice is issued in an attempt to determine the level of interest among qualified VETBIZ registered Service Disabled and Veteran Owned Small Businesses (SD/VOSB) to provide the services described above. Only qualified VETBIZ registered SD/VOSB should respond to this notice. Responses from vendors other than SD/VOSB registered vendors will not be considered. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. The NAICS code is 339113 and the size standard is 750 employees. Interested VETBIZ registered SD/VOSBs are requested to respond to this notice no later than July 17, 2017, 1:00 PM Eastern Time. With your response, please provide a MAXIMUM two (2) page capability statement (additional attachments/pages will not be reviewed) summarizing experience and past performance in fabricating artificial limbs. This should include a list of the limbs provided and your customer information. If currently a vendor under the VA Federal Supply Schedule (FSS) please provide the contract number(s) and listing of Special Item Number(s) (SIN) covered. If not a vendor with a current FSS contract please state this in the capability statement.
